  • Case Study

    The Alohali paper investigates the antecedents (origins) of perceived threats and user resistance to a novel health information technology implementation. The paper focuses on doctors and nurses and the complex reasons these professionals might impede progress of EHR acceptance or produce resistance to implementation. The other study is from a Standford/Harris Poll examining the perceptions of EHR systems among primary care physicians (PCPs). My goal was to have you examine two major perspectives (or biases really) concerning EHRs and professional views of how the technology is received. That of IT professionals looking to improve EHR implementation, and that of physicians utilizing the systems in their daily practices.

    The Alohali paper points out themes of resistance based on concepts such as professional autonomy and hierarchical norms, etc. The Stanford data shows that physicians are most concerned with their workflow and how if affects their face-to-face interactions with their patients. Focusing on these differing perspectives please answer the following questions in 1-2 paragraphs each.

    1. Expound briefly on the five core categories/factors of antecedents that contribute to perceived dissatisfaction and perceived loss of professional autonomy discussed in the Alohali paper (pages 8-11).
    2. Which of these factors, in your opinion, has the greatest weight or influence on successful implementation?

    Overview

    As a health information management (HIM) professional, you face many different and difficult cases that require you to make decisions affecting your workplace and patient care. You must completely understand the laws, policies, and procedures so you can adhere to those regulations when facing these decisions.

    Case Study: Jon is a 42-year-old male who survived a devastating car accident when he was 25 years old. He lives in a skilled nursing facility where he receives full assistance with activities of daily living (ADL). He is in a semi-vegetative state, as documented by one physician. He has no skilled care needs.

    Jons mother was appointed his legal guardian in 1996, one year after his accident. He has three sisters and one brother. For 17 years, Jons mother visited the facility at least twice daily to feed her son. He would eat all his food and did not have problems chewing or swallowing. Jon was a healthy-looking adult male, and he had been medically stable for many years.

    Six weeks ago, Jons mother died of a heart attack. Since then, Jon has deteriorated; he will not take food or drink when the certified nursing assistants (CNAs) offer it. He has lost 20 pounds and was sent to the hospital, where he was treated with antibiotics for pneumonia. After successful treatment of the pneumonia and hydration with IV fluids, Jon was sent back to the nursing facility, but he did not resume eating or drinking except for intermittent small bites.

    The residents attending physician approached the siblings about sending Jon back to the hospital for placement of a gastrostomy tube (a tube surgically inserted into the stomach) to allow for nourishment. Two siblings wish to consent to the surgical procedure. The other two siblings agree that they do not want a feeding tube intervention. They want to allow their brother to go and are advocating keeping him at the nursing facility to die.

    The resident has no advance directives. The facility administrator advises you that one of the siblings is threatening to obtain a court order prohibiting the surgical insertion of a gastrostomy (feeding) tube. What actions will you take?

    After reading the case study, respond to the following:

    • Assess who has the legal right to make decisions on a patients behalf. Compare and contrast end-of-life care decisions based on state laws.
    • Appraise how healthcare power of attorney applies to the patient.
    • Evaluate how ethical principles apply to this case.
    • Recommend HIM policies and procedures needed to ensure that the number of incidences like this is minimized in the future.

    This assignment will help you understand legal issues and policies and procedures in healthcare, which you can incorporate into your final project.
