    Social services is often a challenging role to fulfill in a nursing home. The social services staff deals with a variety of issues from missing socks and broken glasses to unhappy residents and angry family members. Chapter 9 of your textbook offers a case study at the end titled The Pyjama Man. We will use this case study for our discussion board this week.

    As you know, residents are often very independent individuals that have lived their lives in a certain way for a very long time. Often these habits and routines do not mesh well in the nursing home for a variety of reasons to include regulations, facility policies/procedures and the consideration of other residents and their visitors. Read The Pyjama Man case and then answer the following:

    Autonomy is a term that all of you should be familiar with if you are going to work in healthcare and always be mindful of how you protect an individuals right to autonomy while a resident in your facility. But it also just as important to provide quality care, assure quality of life and abide by mandated state and federal regulations.

    Read Case 1, Unhappy with Mechanical Soft Diet, at the end of Chapter 12 in your required textbook. Then reflect on the questions below and respond in an educated, mature and thoughtful manner. At this point I expect some depth to your answers, drawing on things you have learned throughout this course. A simple response will not be accepted.

    After reading the case study, answer the following:

    1. Decisions are often made without including the resident in the process, essentially removing their sense of autonomy. How could this situation have been different from day 1 had the staff included Mr. Lee in the decision-making process? Explore various alternate scenarios/outcomes with each person that made decisions without his input.
    2. If a regular diet is an issue for Mr. Lee (choking, weight loss, dignity, food getting cold, etc), what are some things the facility staff can do to try to do to minimize both facility liability (with regulators for resident decline/risks) and/or also to minimize Mr. Lees personal risks. You can think outside the box or just explore things the staff didnt consider trying. These can be simple ideas of things to do/try or big ideas. Or do you believe the staff did the right thing and have done all that can be done?

    *There is no right answer but you must support your thinking and be insightful in your discussion post. You are exploring options to help him while also helping the facility to avoid citations, trying your best to provide quality care and quality of life for Mr. Lee.

Chapter 5 of Transforming Health Care talks about Virginia Mason’s journey in revamping ambulatory care. In their previous life, the Kirkland facility experienced frustrated patients who complained about long wait times and expressed extreme dissatisfaction with their physician. There is even the case of a fatal mistake due to missed information. Discuss the major, and minor changes, instituted which turned care around for ambulatory care at the Kirkland Facility. 3). Chapter Six in Transforming Health Care, talks about two separate departments, the Ambulatory Surgery Center and the Center for Hyperbaric Medicine. Each undergo a transformation in this chapter. Choose one and talk about the journey that was taken to revitalize the center. What were some of the challenges they faced? What were some of the problem solving methods they used? Which stakeholder input seemed to be the most important and why? What was the outcome? 4). As we come to the end of our journey with Virginia Mason, discuss your main take-aways from this story of transformation. Where have you seen these methods implemented since Virginia Mason put them into play? Where would you like to see them implemented? 5). Discuss the importance of the following concepts in healthcare leaders, and how one can demonstrate the following: * Change Management * Commitment 6). Talk about a team that you were part of that worked really well together. What elements were present that made it an effective team?     The health communication campaign project allows you to apply what we are learning in this course to the creation of your own education program. Students will review one of the Community Needs Assessments listed below, choose one of the topics covered in the report, create an education program to improve health outcomes related to the topic, develop a plan to implement the program, develop a social marketing plan for the program, and create a plan to evaluate the program.

    Each student will complete three separate assignments and consolidate them into one plan for the final project. Specific instructions will be given for each assignment in the Guidelines and Rubrics posted on Canvas. See below for writing guidelines, including the rubric for the entire project.
