    For the wrap-up of this course, it is time to weave together all the knowledge that you have gained. The goal of this project is to integrate what you have learned throughout the course into an interconnected exploration of a specific healthcare policy.

    For the Portfolio Project, you will complete a two-part assignment (further explained below). Pretend that you are making an appearance before a community group that is having a policy explained to them for the first time. Imagine that the group has no knowledge of what happens behind the scenes to create and implement a healthcare policy. You will not actually be required to present this to a group, but prepare the project so that it would be acceptable for an audience of that nature.

    The two documents are as follows:

    1. A mind map (use the WiseMappingLinks to an external site. tool) depicting the essential points and sub-points of your topic. Note: Think of the mind map as a visual outline of your responses to the questions listed below.
    2. A narrative explanation that covers the rationale, research, and processes involved in creating this healthcare policy. The insight gathered should be delivered as a presentation and should serve as the commentary to accompany the mind map if it were being presented to the group mentioned above. The presentation should include a narrative explanation with speakers notes and 8-10 slides (not including your title, references, or the appendix).

    In this Portfolio Project, you will analyze either a federal healthcare policy or a state healthcare policy (it cannot be the same policy selected for any previous assignments). Select a specific federal or state healthcare policy to review. Provide information related to the following points:

    • Explain the initial introduction process of the original policy. Who developed it and/or sponsored it?
    • Assess the research that was conducted and/or analyzed to determine the need for the policy. Why is this policy needed?
    • Does this policy address any specific known disparities?
    • Evaluate the impact, or intended impact, of the policy. Please consider the viewpoints of multiple stakeholders (citizens, providers, government agencies, etc.), including any possible financial impact.

    Note: After you have completed the mind map, take a screenshot and insert the image into an appendix. The appendixLinks to an external site. should be the last page of your document.

    Your presentation should include a narrative explanation with speaker’s notes and 8 – 10 slides (not including title slide, reference slide, or the mind map appendix) and conform to APA guidelines in the CSU Global Writing Center. You must include a minimum of 8 credible, academic or professional references (at least 5 of which must be credible/peer-reviewed), including the text and/or course articles if used in your final project. All credible articles selected must be written within the last 4-5 years, however more recent is preferred.     You work in a 350-bed hospital and have been working on a quality improvement project to reduce AECOPD 30-day hospital admission rates. You pulled 250 patient records and determined that your hospital has a 38% AECOPD 30-day readmission rate, with 28% returning to the ED within 7 days. Of the patients readmitted, nearly 20% experienced a near-fatal outcome requiring admission to the ICU and requiring mechanical ventilation.

    The goal is to reduce AECOPD 30-day readmission rates to 25% over 6 months and then under 20% in the following 6 months, with the long-term goal to reduce AECCOPD 30-day readmission rates to 5% over the following 12 months.

    Through planning & research, your team developed a Risk Stratification Protocol scale to predict AECOPD 30-day readmission risk and then implement any current patient-specific readmission reduction strategies during the first 6 months. The development of a AECOPD Care Bundle Protocol to address COPD diagnosis, emergency care, discharge care, and follow-up/referral care was implemented for the following 6 months.

Your paper should bring together anatomy, physiology, diagnosis, and treatment of the disease. The paper should be 4 to 6 pages long, in APA format.

    You must use at least 4 academic sources which should include online articles, peer reviewed journals written within the last 10 years, one source may be the course text in the construction of your analyses. Two of the four sources must be peer reviewed journal articles. Once summited the paper goes through a plagiarism checker, the paper needs to be under a 30% match in order to not lose points.

    Classmate 1: Diversity in health education design is particularly important because in order to ensure that health messages are accessible, relevant, and respectful to all people, regardless of culture, language, and socioeconomic status, we need to pay attention to diversity. These experiences resonate with the audience and contribute to good health outcomes and equity through integrated health education in a culture of diversity. For example, if language is neglected when health education materials are provided in English only for communities where many people may speak another language, all of this vital information is lost to them. For this reason misunderstandings can develop and health choices are not good. In visuals or examples, exclusion or caricature of particular communities can also drive those communities further away making them less interested in and less trusting the message. Finally, glossing over differences in literacy and socioeconomic status using complex medical jargon or assuming internet access will prevent low income or less-educated individuals from gaining health education, widening the gap. Using cultural humility and an iterative design practice, health educators can develop content that helps to build trust, improves understanding, and reaches different groups of people more effectively. In conclusion, putting diversity at the forefront of health education promotes better health equity and opens the door to better-informed health decision making for all people from diverse backgrounds. It is an essential step toward development of effective and respectful health communications strategies.

    Classmate 2: Diversity matters in health education because people dont all receive or act on health information the same way. What feels clear to one group might be confusing, unrealistic, or even offensive to another. When we design education with one default audience in mind, we risk leaving out the people who may need the message the most. Health outcomes are tied to whether people feel respected, understood, and supported. In medical and public health settings, a lack of diversity awareness can worsen disparities and weaken the patientprovider relationship, which makes it harder to improve outcomes (CHEST Physician, 2024).

    One common miss is language and literacy. If a flyer about managing diabetes is only in English and written like a textbook, many people wont fully understand it. That can lead to mistakes with medications, missed warning signs, and more ER visits. Another example is ignoring culture and daily routines. Nutrition education that recommends foods that are expensive, unfamiliar, or not part of a familys culture can feel out of touch. People may disengage, feel judged, or stop participating. A third example is accessibility. If education is only delivered through small-print handouts, audio-only videos, or in-person classes without transportation help, it excludes people with vision/hearing needs, mobility limits, or rural residents. The consequence is unequal access and bigger health gaps.
