Nursing
Rick is a 72-year-old man previously diagnosed with depressive disorder. He was brought to the emergency room by his wife after he cut his hands with a hacksaw, sustaining damage to various structures and needing surgical intervention. His wife describes that she was going back to work after an extended leave to care for her husband after a recent hospital admission and that this was his first day home by himself. Her son came to check on Mr. Rick and found him on the floor bleeding profusely and called the ambulance.
Mr. Rick retired as an electrician about 8 years ago. Over the past 5 years, his wife describes an increasing indifference to events. For example, when their basement was flooded due to a water leak, Mr. Rick opened the door to the basement, saw the water, and closed the door, saying nothing. This was uncharacteristic of him in the context of his job and training. His wife tells you that the first symptom noted was not caring about things, which could be interpreted as apathy. She notes that the reason he retired early was that he could not keep up with the requirements of his job.
There has been an apparent functional decline. While he is independent in all his activities of daily living, other than cooking, he is dependent on most instrumental activities of daily living. His most recent MoCA score was 16 out of 30. Several comorbid medical conditions are of concern, including hypertension, type 2 diabetes mellitus, and obstructive sleep apnea.
He was assessed for depressed mood by a psychiatrist and started on escitalopram 10 mg daily 1 year ago. There was a mild improvement, but after a few months, he attempted suicide through an overdose. He was admitted to the inpatient psychiatric ward. During that admission, he used shoelaces to tie a ligature around his neck and to the bedroom furniture in an attempt to end his life.
His mood improved over the following few weeks, and he was discharged home with support, including a mental health nurse visiting once a week. His wife had stayed with him at home during the past month, and he was distressed when she left him, even for short periods. He was often tearful even when she was home, and he felt things would never get better. The current episode of self-harm happened on the day she returned to work.

Scenario
Pediatrics ABC is a pediatric practice group in Florida that currently has 50 locations in suburban cities. The practice would like to expand its footprint into rural areas. The Chief Operations Officer (COO) has tasked the Growth and Development Director (GDD) to identify new opportunities to expand by either purchasing existing practices, purchasing land and building a new location, or by leasing office space and renovating.
The GDD learns that the City of Sun Palms, a rural community in South Florida is planning to engage with developers to build residential homes and townhomes, commercial properties including office space and retail, community parks, K-12 schools and medical facilities.
The GDD must conduct due diligence research to identify what healthcare facilities, including pediatric practices exist, what other healthcare services are or will be available, such as diagnostic testing facilities, labs, emergency rooms, urgent care centers, etc., to identify where there are gaps in care or access to care exist.
Research has provided information in the attachments below. Review both before continuining.
Instructions
The GDD is preparing a presentation for the COO. The presentation will include the following information.
- Comparing most urban cities to the City of Sun Palms, a rural city, identify at least three issues of quality in healthcare faced by rural cities and provide examples specific to the City of Sun Palms.
- The GDD must identify which stakeholders in the community could provide insight into their needs.
- In addition to the City Mayor, provide a list of at least three other community stakeholders that should be engaged. Explain why their contribution would be beneficial.
- How would you engage with these stakeholders to receive their input on the community’s needs? Explain why this method of engagement should be successful.
- Based on the research and community engagement, it was recommended that at least one other pediatric practice is needed. Thinking about the healthcare program quality improvement metrics, the next step is to determine what healthcare services should be provided.
- Identify at least three quality improvement metrics or services most important to parents and pediatric patients that would promote patient satisfaction. Explain why these metrics or services are important.
- Identify at least three quality improvement metrics or services that are most important to physician practices. Explain why these metrics or services are important.
- Based on your assessment of the metrics important to patients, those important to physician practices, and the information available in Tables 1.0 and 1.1, what services should the new location offer? Explain and provide justification.
- Based on the services offered, identify at least three clinical, support, and administrative roles that should be hired to staff the new location?
- Thinking about the distinct roles that will be hired and the roles that will directly or indirectly interact with a patient, which role(s) will have a direct impact on Pediatrics ABCs quality improvement program, or more specifically, the patients/parents reviews of their experience with the practice? Explain and provide justification.
Submission Requirements
The GDD will be delivering a PowerPoint presentation to the Chief Operations Officer, Chief Financial Officer, and Chief Medical Officer via Zoom to advise of this growth opportunity, justification for moving forward, and the staffing plan.
- Prepare a PPT presentation with visual and audio recording of your initial research and analysis of the current situation and recommendation for moving forward. Include the above noted discussion points in your presentation.
- The PPT presentation should include at least five slides of content, plus a Cover slide and a References slide.
- Include the speaker notes of your audio recording in the Notes section of the PPT slides.
- Include data, graphics, and pictures to reinforce the main points in support of moving forward.
- Use any software or web ware of your choosing for the audio recording.
- Include the link to the presentation on the last slide of the PPT presentation. If there are any special instructions to access or listen to the presentation, please advise.
- APA 7th Edition formatting is required for the Cover slide, References slide, and any quotes or citations within the PPT document.

Scenario
Pediatrics ABCs leadership team has authorized the opening of two locations in Sun Palms City, FL. They authorized the purchase of the existing pediatric practice and securing a new lease in a shopping center to open a second location. The existing practice was sold to Pediatrics ABC. The purchased location will be named Pediatrics ABC West, and the new location will be named Pediatrics ABC East.
The Chief Operations Officer engaged the Patient Experience Manager and requested that she meet with the onsite clinical, support and administrative team at Pediatrics ABC West to learn about any patient complaints related to the providers, staff, clinical care received, access to care, and general office concerns. The Patient Experience Manager also wanted to learn about any positive feedback received.
An associate from the Pediatrics ABCs Marketing Department identified that current patients/parents from Pediatrics ABC West have been leaving reviews on Google, Yahoo, the locations Facebook page, and reported complaints on the Compliance/Ethics Hotline. The Patient Experience Manager requested that the Marketing Department associate prepare a summary of the types of reviews/complaints and the number in each category received over the previous six months. The practice has been open for 47 years and owned and operated by the same physician.
This practice has 425 patients. 85 people provided reviews in the past six months. Some reviews included more than one type of complaint. Review the attachment below before continuing.
Instructions
- Create a summary report to share the above and following data and analysis with the Chief Operations Officer. The report should be in a Memo template using Microsoft Word. It is recommended you create a table to include the following statistical calculations.
- How many complaints were received in the six-month period?
- Calculate the percentage for each Description category in the complaint table.
- What percentage of patients completed a review or a complaint?
- How many positive feedback reviews were received in the six-month period? What percentage of all reviews were positive reviews?
- How many negative feedback reviews were received in the six-month period? What percentage of all reviews were negative reviews?
- Rank by the most serious types of complaints, from highest to lowest in the table. Separate the positive reviews from the negative reviews.
- Data is the primary driving factor to assist with determining where a quality improvement plan is needed.
- What is your opinion of Pediatrics ABC West from a quality-of-care perspective? Explain your analysis using data.
- Using the above data, identify the most important concern that needs addressing and explain why this concern was selected. This concern will be the focus of the practice locations quality improvement plan.
- A quality improvement team will be needed to assist with developing a quality improvement plan.
- Explain the role of the quality improvement team.
- Identify at least three department heads/associates who should participate in the team.
- Explain your rationale for selecting these team members.
