  • CHE 325 Final Project

This is a 5 page and 5 slide Final project. The Plan of Action must be address. Overview

The final project for this course is the creation of a patient wellness plan and wellness plan presentation.

The role of healthcare and human services professionals is critical in the care of aging individuals. These professionals have the opportunity to offer aging individuals options and strategies to help improve their quality of life and help mitigate the stress and struggle that can come with chronic diseases and other common challenges of the aging population. A professional in a healthcare capacity would explore the different options available to patients in order to help treat and manage their specific health conditions. These healthcare professionals are, in many ways, in charge of the fate of these individuals. They are responsible and capable of not only alleviating some of the issues that come with having a chronic disease, but also improving overall health and wellness of the individuals throughout their life span.

Throughout this course, you will follow a chosen client as they age, and you will try to improve their overall health and wellness. For your final project, you will imagine that you are the healthcare professional tasked with offering recommendations for effectively managing your chosen clients chronic illness(es) and improving their overall quality of life. Your clients primary doctor has already provided a general traditional course of treatment for your clients chronic illness. Your job is to establish a possible health and wellness plan to help with the various factors and challenges that impact the aging population and your individual client in particular. You will recommend other approaches (complementary alternative medicine and lifestyle modifications) that the client could use.

The project is divided into three milestones, which will be submitted at various points throughout the course to scaffold learning and ensure quality final submissions. These milestones will be submitted in Modules One, Three, and Five.

In this assignment, you will demonstrate your mastery of the following course outcomes:

    • Analyze the health and wellness status of aging clients from the perspective of a healthcare professional for informing wellness recommendations
    • Determine appropriate holistic and clinical options for improving chronic disease management in aging clients
    • Recommend wellness strategies informed by established best practices for achieving improved quality of life for aging individuals
    • Communicate the benefits and challenges of various health and wellness options clearly for ethically facilitating informed decision making of aging individuals

    Prompt

Early in this course, you selected a patient to follow. Now that you have monitored your patient, learned of their traditional course of treatment of chronic conditions, and become familiar with their emotional, social, and overall wellness conditions, you will prepare a health and wellness plan for the patient.

Your wellness plan should provide varied approaches to helping your patient improve their quality of life and management of illness. Finally, you will need to communicate your recommendations and options to your patient to help them make informed decisions.

Use the provided to develop your plan of action.

Specifically, the following critical elements must be addressed:

    1. Client Analysis: In this part of the summative assessment, you will analyze the information you have been provided regarding your chosen individual. You will review the overall health status of the individual throughout their life span, key factors that have influenced their health status, and the national guidelines for similar individuals. The audience for this section is not necessarily your client; instead, it should set the stage for your professional recommendations.
    2. Describe the overall health and wellness status of your assigned individual based on materials from the course and your previous analysis. What condition (health, social, emotional, etc.) was the individual in when the course began? What condition is he or she in now?
    3. Base this on materials from the course and your previous analysis.
    4. What condition (health, social, emotional, etc.) was the individual in when the course began? What condition is the individual in now?
    5. Analyze the health status of the chosen individual for different key factors that may have contributed to their current health condition.
    6. For example, what risk factors are typically associated with the health condition of the chosen individual?
    7. Explain the national guidelines regarding the health status of an individual similar to the chosen individual and why they are or are not applicable to the client.
    8. What do the national guidelines (such as, but not limited to, those published by the Centers for Disease Control and Prevention [CDC], Medicare, and the National Institutes of Health [NIH]) state about the specific condition?
    9. Why will these guidelines work or not work for your client?
    10. Wellness Recommendations: In this part of the summative assessment, you will base wellness recommendations for the client on your previous analysis of their health status. Be sure to consider how to help the individual improve quality of life and help manage the individuals chronic disease. The audience for this section is not necessarily your client but should be written professionally with citations where appropriate to support the best practices and theories used to make your recommendations.
    11. Describe the traditional medical approach that was initially recommended for the client and explain why that approach has been ineffective in managing the clients chronic disease.
    12. What factors may have contributed to the ineffectiveness of this approach?
    13. If a specific wellness approach was not given for your client in the information provided, use scholarly research to support your explanation of what would have been a likely medical approach to address your clients health needs.
    14. Determine if a complementary alternative medicine (CAM) approach could be implemented in order to more effectively manage the clients chronic disease and improve their quality of life.
    15. Be sure to explain why this would or would not be recommended.
    16. What would be the benefits or drawbacks of applying a CAM approach to this specific chronic disease?
    17. Consider the many different CAM approaches that may apply to the situation.
    18. Determine if lifestyle modifications could be implemented in order to more effectively manage the clients chronic disease and improve their quality of life. Be sure to explain why this would or would not be recommended.
    19. What are some changes, both small and large, that the individual could make to improve their chronic disease?
    20. For example, are there dietary changes, changes to an exercise regimen, medication changes, and so forth that would benefit your client and create a more balanced lifestyle?
    21. Recommend wellness strategies informed by best practices for addressing wellness issues of the chosen individual. Be sure to incorporate CAM approaches and lifestyle modifications if appropriate and cite your sources.
    22. What accepted wellness strategies could you recommend for your individual?
    23. How would you adapt the wellness strategies to better fit the needs of your individual?
    24. Be sure to use scholarly resources to find examples of wellness strategies/recommendations for someone with similar health concerns.
    25. Use the provided template to develop a plan of action for your specific individual to implement these strategies and achieve optimal health, justifying response. Be sure to consider both how the individual needs to manage their chronic disease and how they can best address wellness issues.
    26. Explain how your recommendations will help the individual manage the individuals chronic disease and improve quality of life not only in the present but also in the future. Be sure to justify your response with research.
    27. Presentation: In this part of the summative assessment, you will develop a slideshow presentation to your client explaining the benefits and challenges that they would have to consider when making decisions about your recommendations.
    28. Communicate the benefits of the health and wellness recommendations of the chosen individual clearly and within the bounds of ethics.
    29. This is not merely a repeat of your wellness recommendations but communicates the benefits of your client adopting the wellness recommendations you have given them.
    30. Articulate the challenges of the health and wellness recommendations to the chosen individual clearly and within the bounds of ethics.
    31. Give specific examples of the challenges your client might face if they engage in the health and wellness recommendations you have provided.
    32. Communicate how the recommendations will be more effective than other options. Discuss all aspects ethically necessary and provide examples to support your response.
    33. You can convey this information visually as well as through your writing. Be sure to provide examples to support your response.
    34. Why should the individual choose your path as opposed to others?
    35. What makes your plan the best plan?

Directions/How to Use this Template: The content in this table does not have to be written in full sentences and can be presented in the form of bulleted lists. Once you have filled out this template, you will use it to inform the written narrative of your wellness recommendations plan, in Milestone Three.

Specific Health Domain: Identify the specific health domain with a concern to be addressed (e.g., social, psychological).

Recommendations: What would you recommend (e.g., tangible activities or action steps) to address the concern?

Resources Required: What resources would be needed to follow up on your recommendation?

Order of Implementation: In what order should your recommended actions be implemented? For example, should your client work on improving his or her diet or exercise first? Are there benefits and challenges with one order of activities over another?

Desired Outcomes (Improvement Measures): What is the desired outcome of your recommendation? For example, is it that the client will have a better diet? Think short- and long-term goals and overall quality of life.

Justification: What is the rationale or justification for your recommendation? For example, is there a reason a different (perhaps a more common or popular) plan would not work for your client, thus influencing your proposal?

It is a rare opportunity to speak to a health services professional and glean insights from his or her experience. Gathering another health services professionals insight and reflecting on this experience will better prepare you as you enter the field. In this activity, you will find and select a local child welfare service provider or a child protective service worker to interview. For example, you may interview a professional within a foster care placement agency, adoption agency, or respite care agency. You can also interview a child welfare service worker from a local child protective services agency. • Motivation interview

The “spirit” of Motivational Interviewing is brought to life through four specific micro-skills, known by the acronym OARS. These are not just communication tools; they are the “rowing” mechanism that moves a client through the process of change. Let’s put this into practice.

    The Prompt

    Read the following client statement:

“I know I should probably stop drinking so muchit’s affecting my sleep and Im always late for work. But honestly, its the only way I know how to unwind after a stressful day, and all my friends go to the bar after our shift.”

Your Task: Provide a response to this client using each of the OARS components. Label your responses clearly:

    1. Open-Ended Question: Craft a question that invites the client to explore their ambivalence further without feeling judged.
    2. Affirmation: Identify a strength or a positive value the client demonstrated in their statement and affirm it.
    3. Reflective Listening: Write a “complex reflection.” Instead of just repeating what they said, try to reflect the underlying feeling or the “change talk” you hear (e.g., “On one hand… but on the other…”).
    4. Summary: Write a brief summary (2-3 sentences) that ties these elements together, ending with a “check-in” question to see if you understood them correctly.

    The Follow-Up Challenge

After you provide your OARS examples, answer this: Which of these four skills do you find the most difficult to implement, and why? (e.g., Is it hard to avoid “Closed” questions? Is it difficult to find something to affirm?)

    Submission Requirements

    • Initial Post: Due Thursday. Ensure your OARS examples are distinct and labeled.
    • Peer Response: Due Sunday. Review a classmates Reflection. If you were the client, how would that reflection make you feel? Suggest one way to “deepen” their reflection to highlight the clients internal motivation.

    OARS Cheat Sheet for Success

Quick Reference for SkillsSkillPro-TipExample StartOpen-EndedAvoid “Why” (it can sound accusatory). Use “What” or “How.””What would be different if…”AffirmationMust be genuine. Focus on character or effort, not just “good job.””It takes a lot of self-awareness to notice…”ReflectionTry to make a statement rather than asking a question.”It sounds like you value your job, yet…”SummaryUse “transitional” summaries to move the conversation forward.”Let me see if I have this right…”

Here is a model response to the client scenario. You can provide this to your students as an “Exemplar” after the discussion closes, or use it as a grading rubric to see if they are hitting the nuances of each skill.

    Client Statement Recap:

“I know I should probably stop drinking so muchit’s affecting my sleep and Im always late for work. But honestly, its the only way I know how to unwind after a stressful day, and all my friends go to the bar after our shift.”

    Model OARS Response

    1. Open-Ended Question

“What are some of the things youve noticed about how your work life might change if you had more energy in the mornings?”

    • Why it works: It avoids a “yes/no” answer and directs the client to imagine a positive future (Change Talk) rather than focusing on the “bar.”
    1. Affirmation

“It takes a lot of honesty and self-awareness to recognize that your current way of unwinding is starting to get in the way of your professional goals.”

    • Why it works: It validates the clients character (honesty) rather than just “approving” of their behavior.
    1. Reflective Listening (Complex Reflection)

“On one hand, the bar is where you feel connected to your friends and find relief from stress, but on the other hand, youre starting to worry that the cost of that relief is your reliability at work.”

    • Why it works: This is a “double-sided reflection.” It acknowledges the “pros” of the behavior (unwinding) while highlighting the “cons” (work issues) without sounding judgmental.
    1. Summary

“To make sure Im following you: you value your friendships and need a way to decompress after a hard shift, but youve also noticed that drinking is interfering with your sleep and your performance at a job you care about. Have I missed anything important?”
