    Practice Change and Stakeholder Communication Strategies

    Hypertension remains one of the most prevalent chronic health conditions in the United States and is a major contributor to cardiovascular disease, stroke, and kidney failure. African American adults experience disproportionately higher rates of hypertension, poorer blood pressure control, and greater hypertension-related complications compared to other racial and ethnic groups. The practice problem identified at the practicum site is the need for implementing a staff education program to improve hypertension management in African American adults in primary care. Improving staff knowledge and strengthening evidence-based hypertension management strategies can help reduce disparities and improve patient outcomes in this population.

    Effective collaboration with stakeholders was essential in gaining support for this practice change initiative. Stakeholders included nurse practitioners, physicians, nurses, clinic administrators, and support staff within the primary care setting. One of the key communication strategies used to gain cooperation was early stakeholder engagement, which involved discussing the problem, presenting data on hypertension disparities, and explaining the goals of the proposed staff education program. Transparent communication helped build trust and encouraged stakeholders to participate in the planning process. Engaging stakeholders early also allowed team members to provide feedback on workflow considerations and potential barriers to implementation. Research suggests that early engagement and shared decision-making improve staff acceptance and sustainability of practice changes (White et al., 2021).

    To build and maintain an effective team, several strategies will continue to be used throughout the implementation process. These strategies include establishing clear roles and responsibilities, maintaining open communication channels, and conducting regular team meetings to review progress and outcomes. Encouraging interdisciplinary collaboration will also support the success of the project, as hypertension management often requires coordinated efforts among clinicians, nurses, and administrative staff. Strong teamwork and collaboration have been shown to improve healthcare quality and patient outcomes by fostering shared accountability and collective problem-solving (Boamah et al., 2022).

    During meetings with stakeholders, different communication styles were observed. Some stakeholders demonstrated a collaborative communication style, actively sharing ideas and participating in discussions regarding patient education strategies and workflow improvements. Others used a more task-oriented communication style, focusing primarily on operational concerns such as time constraints, documentation requirements, and staffing considerations. Recognizing these communication styles helped guide the approach used to engage partners effectively. Strategies such as asking open-ended questions, summarizing discussion points, and encouraging quieter participants to contribute helped ensure that all stakeholders felt included in the conversation.

    Reflecting on the stakeholder engagement process, one effective strategy was presenting evidence-based data related to hypertension disparities among African American adults, which helped illustrate the urgency and importance of the project. Additionally, practicing active listening allowed stakeholders to express concerns and suggestions, which strengthened collaboration and trust within the team. In the future, strategies such as structured meeting agendas, follow-up communication, and continuous feedback loops will be used to further enhance team engagement and ensure sustained communication. Effective nurse leaders must continually refine their communication and leadership skills to support collaboration and facilitate successful implementation of healthcare improvements (Wei et al., 2023).

    In conclusion, implementing a staff education program to improve hypertension management among African American adults in primary care requires strong leadership, effective communication, and stakeholder collaboration. By engaging stakeholders early, fostering open dialogue, and utilizing inclusive communication strategies, nurse leaders can build effective teams capable of implementing and sustaining meaningful practice changes that improve health outcomes and reduce disparities.
