  • Need help with Clinical Care Plan

    -Fill out each section of the care plan for privacy and HIPAA concerns, do not place any identifying information such as Name, DOB, phone number, MRN/EHR #, etc.

    -Fill out the entire nursing care plan form, but if there is no data to support a section, place an “NA”. Under Abnormal labs/diagnostics, include information pertinent to the patients current problems. For example, increased GFR due to trauma related to falls or abnormal CXR with infiltrates in the right lung from aspiration pneumonia. Explain why you think that the lab value is abnormal as it relates to the patients current problem.

    -For the H&P section, summarize a brief synopsis of the patients significant history, current problems, and admitting diagnosis. A good place to find this information is usually in the admitting/ER consults doctor’s notes.

    -All documentation (references and citations) will be done in the current 7th Edition APA format. Will not accept references from Wikipedia, Nurselabs.com, or any site that is of a private opinion. It must be vetted and verified by trusted sources such as peer-reviewed journal articles, published Journal sites, or government sites such as the National Institutes of Health (NIH) or the Centers for Disease Control and Prevention (CDC). Also accepted are accrediting bodies (Joint Commission, CCNE, ANA) or published books.

    Remember to use your resources when you do this, such as your nurses and doctors, and talk to the patients to better understand why they are there from their perspective. Remember, you are to learn and interact with as many people as possible while you are on rounds at the hospital.

    Use SMART Goals, interventions, rationales, and evaluations that can be measured within a shift. Also, remember, don’t make a care plan complex. Please keep it simple.   • Microbiology

    A 94-year-old woman went to her local hospital emergency department in mid-November 2001 complaining of a 5-day history of weakness, fever, nonproductive cough, and generalized myalgia (muscle aches). Otherwise, for a person her age she was fairly healthy, although she did suffer from chronic obstructive pulmonary disease, hypertension, and chronic kidney failure.

    On physical examination, her heart rate was above normal and she had a fever of 102.3F (39.1C). The rest of her physical examination was normal. Initial laboratory studies of blood cell count, blood chemistries, and chest X ray were also normal except for the chemical urine testing. This finding along with the fever suggested an infection, so the patient was admitted to the hospital. Samples of blood and urine were sent to the microbiology laboratory and set up appropriately.

    The next day, microscopic evaluation of the urine culture revealed rod-shaped bacteria that stained red, and the blood culture revealed rods that stained purple. The liquid blood culture was then transferred to appropriate solid media. This finding in the blood was unusual, so a sample culture was sent to the state health department laboratory. Antibiotic therapy was adjusted, yet the patients condition deteriorated. Her most serious symptoms localized to her chest, and she was transferred to the intensive care unit. Four days after admission, the health department announced that the bacteria found in the patients blood were Bacillus anthracis. She was suffering from inhalation anthrax. Further testing showed these bacteria to be of the same strain that had been involved in the recent bioterrorist attack. Despite treatment, the patient died on the fifth day after admission. Mother issue: risk for ineffective, postpartum recovery related to culture, postpartum confinement practices, and possible delays seeking care. – basically this culture when the mother gives birth, they stay home for 30 days not leaving the house because in their culture thats really good for recovery and protection however, its causing like if there was an emergency and shes having heavy bleeding shes not gonna go leave the house because shes on that 30 day homebound situation.
