Scenario for Part 2 of Course Project

You are continuing to prepare for the upcoming global health conference where you will present a poster presentation. In part 1 of your project, you chose a significant global health issue as your focus and researched the incidence, prevalence and implications. Now, you will explore how determinants of health impact your identified global health issue.

Story 1 One issue that has ethical, legal, and accreditation implications for my practice as an Informatics Nurse Specialist (INS) is telehealth neurological assessments for suspected stroke patients in the emergency department (ED), especially when workflow gaps occur. In a busy ED, neuro carts are sometimes misplaced, not charged, or stored in another pod, which delays connection with the remote neurologist and slows NIH Stroke Scale completion. Those minutes matter when determining eligibility for thrombolytic therapy such as tenecteplase, and delays directly affect door to implementation times of stroke certification standards from The Joint Commission and clinical guidance from the American Heart Association. Compounding this, my hospital still relies on paper documentation for TNK administration. Paper forms increase the risk of incomplete time capture, missing contraindication checks, scanning delays, and documentation that is not immediately visible to the interdisciplinary team. From an informatics perspective, this creates legal exposure and quality reporting challenges because time stamps, dosing verification, and eligibility criteria are not structured or easily retrievable for audit. In practice, I would standardize cart placement, explore real time location tracking, embed structured NIHSS and TNK eligibility tools directly into the EHR, and transition from paper to electronic stroke documentation with mandatory fields and automated time stamps. I would also partner with ED leadership to review door to intervention metrics and reinforce rapid stroke alert activation processes. Aligning technology, workflow, and documentation strengthens patient safety, supports accreditation compliance, and reduces organizational risk. Story 2 Since the end of 2020, many rural hospitals have either stopped delivering babies or announced they will do so before the end of 2026. This represents a 10% reduction in rural labor and delivery units. The accreditation issue in rural areas is a significant concern given the high costs of labor and delivery services. Rural hospitals often face financial challenges, including high fixed labor and delivery unit costs, which can lead to the closure of these services. To build a collective response, it is crucial to prioritize workforce training and education. Preparing physicians, advanced practice providers, nurses, dentists, and other healthcare professionals to work in rural settings can strengthen maternal health services. Strategies such as Grow Your Own and Career Ladder Programs, along with investments in rural healthcare education, can support recruitment and retention efforts. Interprofessional education and telehealth training are vital tools for addressing staffing challenges faced by providers and hospitals operating in isolation, thereby encouraging collaborative solutions

Evidence Based Practice Paper

This assignment focuses on the integration of evidence based practice (EBP) and research in health care. EBP is an integration of research evidence, clinical expertise and a patient’s preferences. This problem-solving approach to clinical practice encourages health care providers to provide individualized patient care. The inclusion of EBP in nursing provides nurses with the scientific research to make well-founded decisions. Through EBP, nurses and providers can stay updated about new medical protocols for patient care. By searching for documented interventions that fit the profiles of their patients, nurses can increase their patients’ chances for recovery.

EBP enables nurses to evaluate research so they understand the risks or effectiveness of a diagnostic test or treatments. The application of EBP enables nurses to include patients in their care plan. This allows patients to have a proactive role in their own healthcare since they can voice concerns, share their values and preferences and make suggestions on how they want to proceed.

EBP strategies allow nurse practitioners (NPs) and other health care providers to translate research findings into clinical practice. With efficient literature-searching skills and the application of formal rules of evidence in evaluating research findings, providers can apply existing scientific knowledge in their clinical practice for each individual patient.

Directions for Paper Assignment

3. Based on the evidence what process, structure, or outcome measure related to your POI will you expect to change in your practice? You can choose management of a disease process, a protocol that needs updating, a process change, additional diagnostic tools and procedures…. Choose a topic that will improve patient outcomes, reduce costs, or save time in treatment protocols (just some ideas).
4. Identify a change theory you will use. Discuss the specific parts/steps of change you will use in advocating a change in practice. To review change theories, please refer to Week 4 material.
5. Search the current literature (last five years) for two research articles related to the selected POI/topic and what area you are proposing change. One article must be quantitative research and one must be qualitative research.
6. Briefly discuss the following for each article.
7. Purpose, research design, and methodology of studies (steps, procedures, and strategies for gathering and analyzing data)
8. Data collection. Describe the procedures developed to guide the collection of data for each study, and sampling of participants.

This is my prior discussion from the same class, I hope it can be of use:

I proceed to present how I perceive the major domains or meta-paradigms of nursing.

First, a Person is a being composed of a biological sphere and a mental one that can be further divided into emotional, spiritual, psychological, etc., just like a body has systems within itself. A Person is the mind and the body combined; the objective and the subjective. Secondly, Health is more like a label to describe the state of these spheres that form a Person. If a sphere is in harmony and balance, then I call it healthy, and if it is altered or affected negatively, I call it unhealthy. I define the Environment as the factors external to the physical body of a person. Environmental factors should be seen as potentially modifiable to improve the Health of the patient. Finally, Nursing is a relationship established with the goal of assisting the Person to achieve health while respecting ethical standards.

These major domains interact with each other; thus, the negative alteration of one could also negatively impact another. For example, a terminal disease diagnosis pertaining to the persons biology likely would affect the psychological part of the Person, or a very contaminated environment could cause negative biological consequences. All three concepts are intimately connected and influence each other.

When looking at Watsons theory, the concept of Person is really similar, but she goes into further subdivisions of the subjective area, so instead of being only mind, she says there is a mind and also an independent spirit/nature, which is not contained within the mind but is outside of it. When analyzing the environment, I remain a generalist by seeing it as all outside of the body, but Watson goes further and specifies it includes the non-physical and even the nurse itself. Watsons view of Health and mine hold pretty similar perspectives. Finally, her concept of nursing is very well developed, introducing the 10 caritas processes that focus on love, while my concept is more geared towards a relationship respecting ethical standards.

When comparing my theory to that of Watsons, I see many points in common but also have slight differences. I believe those similarities are caused by us being contemporaneous in a society that has been influenced by Greek philosophers like Aristotle, who introduced concepts like body and mind in balance, or even the Asian culture concepts of Jing and Yang, which emphasize balance as the base of harmony and well-being. Even more, when going through her theory, I couldnt avoid feeling an instinctive understanding, as if her concepts were embodying part of a shared cultural consciousness in America.

Regarding the future of nursing, I cannot avoid feelings of concern. With the increasingly aging population, decline in natality rate, and the ever-increasing lifespan, the demand for nurses is guaranteed to increase to levels never seen before in a field where understaffing is a permanent issue. This will increase the pressure on nurses to assume the care of more patients at a time and thus increase the risk of negative outcomes for the patients. On the positive side, I see technology as an aid that could take some of the workload away from the nurses shoulders with programs that speed up the charting process or even additional staff that would handle tasks like recounting patients belongings that only hinder nurses from focusing on more complex and vital tasks.
