    Information & Guidelines:

    Nurses in various healthcare settings work with clients with acute and chronic health conditions. It is critical for the nurse to know how to address the needs of these vulnerable clients and their families competently. This requires nurses to know the underlying causes and manifestations of a variety of common medical diagnoses and their impact on client self-care, individual development, and family function. This assignment will give students the chance to explore one acute or chronic condition affecting adults and discuss the nurse’s role in caring for clients with these conditions.

    • Each student will select a chronic health diagnosis that impacts adults during the first week of the term. Topics may include but are not limited to: Asthma, COPD, Heart Failure, CVA, Type 2 Diabetes Mellitus, BPH, Acute Kidney Injury, and Pneumonia
    • Students will conduct research into the specific disorder and complete a professional paper about the condition. Details about content are noted below.
    • Students will follow the KCC Nursing Program Written Paper Guidelines for professional papers (cover page, APA (American Psychological Association) format, etc.) as well as the Written Paper Policy for submission as stated in the Nursing Program Student Handbook. The professional paper is a research paper, therefore multiple peer-reviewed, evidence-based resources should be utilized to produce the document. It is expected students will synthesize references and sources into all components of their papers and not just utilize their textbooks and/or one singular source for any one component of the paper.
    • The ability to synthesize information into one’s own words is a hallmark of professional writing and is an expectation on this professional paper and subsequent discussion board posts. Paraphrasing your sources, as opposed to direct quotation, should be done whenever possible to demonstrate that you understand what the author of your source was trying to articulate and can share what you learned in your own words.
    • Introduction: In 1-2 paragraphs, introduce the acute/chronic health condition. Include current data regarding the prevalence of the disorder nationally and globally.
    • Body of the paper: The following content must be included: 1.) common manifestations and causes of the underlying acute/chronic medical diagnosis; 2.) common manifestations of an acute condition and/or exacerbation of a chronic condition; 3.) psychosocial issues related to the diagnosis including the potential impact of the acute/chronic medical condition on client self-care, individual development, and family function; and 4.) at least four common nursing diagnoses associated with the acute condition and/or exacerbation of the chronic condition.     After choosing your Healthy People 2030 topic for your final project, lets begin to think about how preventative care should play into the fulfilling of the Healthy People 2030 goals.

    For this weeks discussion, consider your chosen Healthy People 2030 goal and choose a fictional family (at least 4 family members) that deals in some way with your chosen goal.

    Using chapters 6 & 7 (Edelman & Kudzma, 2022, pp.145-201) find examples of nursing interventions and formulate an informal plan of care for addressing the chosen issue within your fictional family group. (Feel free to name your family member, provide ages, and any background that you think might be pertinent to their situation.)

    Think about the following areas as you write your plan of care as a discussion post:

    • Page 165 in the Edelman & Kudzma (2022) text provides an overview of the process for formulating a care plan.
    • Which members of your fictional family will be most affected by the goals issue?
    • How will you educate the family concerning your chosen goal?
    • How will the family plan for the future as other family members may become susceptible to the same issue(s)?
    • What interdisciplinary partners might you consult or refer to in aiding your patients to progress with the chosen issue?
