    Think back to meeting Justin Jones from Module 5. He was admitted after a motorcycle accident. He had surgery for fracture reduction on his legs and now has casts on both legs and one arm. For the past three days, he has been receiving narcotic analgesics for severe pain.

    Today, he has a new priority problem: constipation. The nurse enters the diagnosis into an electronic care plan. In addition to a list of suggested assessments, the computer database generates the following list of suggested interventions:

    • Institute a program to establish a regular pattern of bowel movements
    • Administer laxative or stool softener, as prescribed
    • Administer enema
    • Remove stool manually
    • Encourage increased fluid intake, including warm liquids; instruct on and encourage a high-fiber diet
    • Encourage a regular program of activity and exercise
    • Perform manual reduction of rectal prolapse

    My response:

    The constipation experienced by Justin Jones is most probably connected with opioid consumption, lack of mobility, and decreased post-operative fluid consumption. As a nurse, I would first opt to use noninvasive interventions and then subsequently proceed to more aggressive interventions. Some of my priority interventions would involve the promotion of increased fluid intake, high-fiber diet as tolerated, and the establishment of a regular bowel routine. These strategies promote normal bowel movements and they also prevent additional complications without causing unnecessary discomfort.

    Since Justin is taking narcotic analgesics, I would also prescribe a stool softener or stool laxative as a second drug, because opioids greatly decelerate gastrointestinal motility (Horrigan et al., 2023). his intervention is a direct response to the cause of his constipation and is regarded as a common preventive treatment of opioid-induced constipation. Moreover, I would encourage activities and exercises within his restrictions, including repositioning in bed and upper-body range-of-motion activities, as immobility exacerbates constipation.

    More invasive interventions like the use of enemas or manual stool removal would be used as the last-line therapies in case of failure of the conservative treatment, since they may be uncomfortable and can cause harm particularly in older adults. In general, my practice is aimed at prevention, comfort, and solving the root causes of constipation as well as preserving patient dignity and safety.

Encouraging Justin to increase his fluid intake, eat high-fiber foods as tolerated, and establish a regular bowel routine aligns with the first-line recommendations outlined by the Mayo Clinic.     Peer#2:Constipation is a common side effect of surgery due to the anesthesia administered and pain medication. The nursing interventions that I would choose for Mr. Jones are.

    Institute a program to establish a regular pattern of bowel movements. It is essential to educate Mr. Jones to schedule a time each day to sit on the toilet for about 15 minutes to attempt to have a bowel movement. It is crucial to. Be patient and consistent with this exercise, as it can take a few days to establish a pattern. This and increasing fluids and high fiber duet should be our first intervention, and if it is not helping, we could incorporate laxative administration.

    Administer laxative or stool softener, as prescribed. These require patience as they usually take from 1 to 3 days to work in the system. This will help stimulate bowel movements. These can be taken with the pain medication.

    Encourage increased fluid intake, including warm liquids; instruct on and encourage a high fiber diet. Increasing your fiber intake helps in creating bowel movements. These foods include whole grains, fruits, and vegetables. Secondly, fluids will allow your stools to soften. Additionally, warm liquids will help in creating bowel movements, thought they will be watery.

  • Research and EBP Proposal

    Instructions for the Basic Research or EBP Proposal To fully appreciate and gain a deeper understanding of the research and EBP process, students will write a six-to-seven-page basic research or evidence-based practice project proposal. The paper will be completed in drafts and submitted along with drafts of the Evidence Analysis Grid (EAG). Peer review and the EAG will be used to assist in writing the proposal. The review of the literature will be the first draft. The proposal will be typed in APA format including the following components. The Methods section may be bulleted. Initial steps: A. The first step is to identify a problem and create a PICOT question and use the terms in the PICOT as your search terms for your data base searches. Keep track of your search terms, the number of hits received, the number of articles reviewed, the number included and why, and the number rejected and why. (I have provided the Johns Hopkins instrument in the folder on Bb for this assignment to help you track this information). B. Include the essential information from each article included in your EAG. C. Assess the quality and quantity of your evidence. What does the literature say about the problem? Is there is a good amount of quality evidence that is consistent in the outcomes? If Yes you have an EBP project and you will be completing an EBP project proposal. If the answer is No, or if there is inconsistency in the literature, this means there is a gap in the literature, and you will be completing a research proposal. If you have a research proposal, then you must determine if a qualitative or quantitative design is the best way to approach the problem. Significance State why this proposed study/project is needed. This is the so what factor. If you cannot clearly and logically state the importance and value of the study/project, then why consider it? Definition of Terms – Define the major terms/concepts in your study/project theoretically and operationally. A theoretical definition is the definition of the term or concept according to your intended use in the proposal. The operational definition is how the term or concept will be measured. Example: Depression (you could use the DSM definition with citation) or your own definition as to how you intend to use the term. This would be your theoretical definition. Depression will be determined as a score of 9 or above on the PHQ-9 (citation) for the purposes of this project. This would be your operational definition because this is how you are operationalizing (measuring) the concept of depression. The Background through to the Definition of Terms should encompass one or two pages of the paper. This section of the paper is worth 15%. Theoretical Framework-Explain the theoretical framework or EBP model for this project. Please note, theoretical frameworks/models are not usually included in qualitative research studies because qualitative research usually generates theory. However, for the purposes of this paper, everyone will include a theoretical framework or EBP model. Explain this in one paragraph. 10% Literature Review Introduction Use this section to introduce the reader to the review of the literature. Include the search terms you used to conduct your search, the number of hits you received, the number of articles reviewed, the number included in the review, and explain why they were included, and the reasons for the excluded articles. This is usually one paragraph. You could also demonstrate this and eliminate some of the narrative using a PRISMA flow diagram. 1. All sources should be peer reviewed and published within the previous 5 years. You may include: 1. One clinical article 2. One expert opinion (position statement, clinical practice guideline, lit review, scoping review, integrative review) 3. One publication may be older than 5 years 4. The rest must be research publications – ideally of the highest level of evidence if you are considering an EBP proposal or multiple types of research studies if you think you will be doing a research proposal. Body of the Literature Review- In this section briefly discuss the literature reviewed including only the most salient information. Try not to present each article separately. It is better to integrate and synthesize the information so the reader can clearly understand how the evidence you are presenting, supports the proposal. If you have different focused areas, identify them using subheadings. Summary Summarize/synthesize the literature reviewed. Synthesize it so it justifies the need for the project you are proposing. This should be about a paragraph, but can be a little longer if necessary. This section should be at about two pages of text. 30% Methods This section of the paper may be bulleted due to the time constraints in this class. Design- State the design of the study/project, and explain the rationale for the design chosen. Examples for research proposal designs include Qualitative Research using a phenomenological method, or another design might be Qualitative using Grounded Theory, etc. Quantitative Research using a descriptive survey; Quantitative using a cross-sectional design; Quantitative using an experimental method; Quantitative using quasi-experimental method; etc. If you plan to use both methods it would be a Mixed-methods design.
