General Instructions

The purpose of this discussion is to examine an assigned disease process and determine which disease process explored in the discussion Bayani, the client, is experiencing. Follow the instructions below and your faculty member will reveal which disease process Bayani is experiencing at the end of the discussion week.

Step 1: Review the following client scenario:

Bayani, a 62-year-old male client, presents to the NP with abdominal pain with his wife. Bayani is mildly confused, and according to his wife, he is normally fully oriented. His abdomen is soft and non-distended, and his wife states he has been urinating and drinking more water than usual. She also states he has had foul-smelling urine and is concerned he also has a urinary tract infection.

Step 2: Review your assigned disease process from the chart below. Your assigned disease process is based on the first letter of your last name.

Condition: Peptic Ulcer Disease (PUD)

Step 3: Review the client case scenario and answer the discussion prompts below with explanation and detail, providing complete references for all citations.

Include the following sections:

Application of Course Knowledge: Answer all questions/criteria with explanations and detail.

1. Discuss the underlying pathophysiological mechanisms of your assigned disease process. Which clinical manifestations observed in Bayanis case could be explained by the pathophysiological mechanisms?
2. Analyze Bayanis clinical manifestations in the context of your assigned disease process. Do these findings support a diagnosis of your assigned disease process? Why or why not?
3. Identify and justify the diagnostic tests (including labs, imaging, or other diagnostic tests) that would be most appropriate for investigating a diagnosis of your assigned disease process in Bayani. What could the results of these tests look like in your assigned disease process?

Chronic disease management

We use care plan meeting to collaborate with patient and family. This is done by the IDT- interdisciplinary team. This has Nurse, dietician, recreation, pharmacy, MD, and social work. We talk about their drug regime and see if pharmacy can continue what they have been taking. We assess also how much the patient or the family understands the medication and why they are taking it.

A lot of time we know that these patients may go through a culture shock transitioning from the hospital to the rehab facility. So aim to answer all the questions to make them as comfortable as we can. Keeping into account the different rules and regulation that are in place for the nursing home compare to other facilities. CMS/DOH.

We work together and coordinate the care plan because it allow all those who are involved with the patient to be on the same page and it gives that patient care center. It allows everyone to address the needs of the patient. In the facility we drain the Pleurex catheter in the morning for everyone, however, this patient requests in the evening because they sometimes get tired and exhausted depending on the amount that has been drawn out. We agree to do that resident draw in the evening as per their request because it is all about the patient and the need to help them rehab and return home.

Wellness and Disease prevention

During this week I learned a lot about plan of correction for the Department of health. Knowing the F-tags and what regulation is telling you to do, then educating your staff of that policy. The facility is given a certain amount of time to fix these corrections and document the change. If all goes well and the department of health accepts the plan of correction, then the facility can continue to function while waiting for another visit anytime. Collaborative of care is needed to fix this because no one manager, supervisor or director can complete this task. Being that the facility has over 300 employees, one must educate everyone there and document it. Then going forward they must create a new policy for that incident not to occur again and the repercussions if it did. Every citation from the surveyor must be corrected and competency of the employee must be documented. This whole process brings stress for both the manager and staff.

We focus on hazard prevention- are we transferring the patient correctly by looking at their transfer status and following their care plan.

Infection control- We are assessing the patients skin and caring for them to prevent any breakdown in the future.

Is the staffs competency up to date, showing they know how to care for these patients and is informed of the facility policies and procedures as to how they should do so.
