  • Portfolio Research

    Purpose

The purpose of this portfolio piece is to:

    • Apply knowledge and theory related to the prevention, diagnosis, and management of select uncomplicated common acute and chronic health problems for individuals across the lifespan.
    • Develop differential diagnoses and plans of care based on evidence-based research and clinical practice guidelines.

    Overview

In this part of the portfolio assignment, you will choose one of the topics provided and research it to gather reliable information from which you will make a video script. The deliverable for this part of the portfolio work will be an annotated bibliography with an emphasis on summary and evaluation of sources.

Your video will represent a basic but informative explanation of the topic. Even though your final product will use language that most anyone will understand, your work must be informed by the best evidenced-based sources you can find. It will be your job to translate the findings to lay terms. A sixth-grade level is an appropriate target for your wording and explanations. You can use a

to be certain you are at the right level.

Research your topic using library databases, the library guide for FNP including UptoDate, and other credible, evidenced-based sources. Here are the that you may choose from for the portfolio project. If there is a topic not listed that you would like to focus on, please discuss your idea with the instructor for approval.

    Action Items

Topic: Hypertension: Understanding silent organ damage.

    1. Research your topic using appropriate library databases, the FNP library guide including UptoDate, and other credible, evidenced-based sources. You need at least five (5) sources. None of your sources may come from trade publications or sites that end in something other than .gov or .edu.
    2. Your deliverable will be a critical appraisal of the evidence for each source you include. Read the following articles on how to do an appraisal:
    1. Use the wnload template
    2. to analyze and evaluate each source. Repeat the template for each source and combine them into one Word document. Then summarize each source addressing health maintenance, promotion, and epidemiology. For the epidemiology, provide statistics in the communities (when available) county, state, and national. This can be done in a table. Provide a references page. Your summaries and reference page must be in APA format and style.

  • Community-based support assessment and identifying gaps for…

Writing required only for 1-2 part of the assignment.

    Part 3A: Community Assessment

As part of the capstone project, develop a detailed plan for conducting a community assessment, identifying gaps, and proposing a suitable intervention (Part 3B). While you will not carry out the assessment or intervention, your task is to create a comprehensive and theory-informed assessment plan.

Assignment Sections 1. Community Identification and Definition (~1-2 paragraphs)

In this section clearly define who the community is and establish the boundaries that will guide the assessment. Required Content

    • Type of community (identify your at-risk, marginalized or vulnerable population clearly) Geographic boundaries (if applicable); include a map if required as an Appendix.
    • Population characteristics (e.g., age distribution, cultural or linguistic groups, income, housing, migration status)
    • Community context (e.g., urban/rural/remote, historical or structural influences)

    Guiding Questions

    • How is this community defined?
    • Who is included and who may be excluded by this definition?
    • How does this definition shape what can be assessed?

Choose your population from the sample list of at-risk, marginalized or vulnerable population.

Marginalized and At-Risk Populations (Broader groups affected by structural, social, economic, or systemic inequities)

    • Racialized communities
    • 2SLGBTQIA+ individuals
    • Newcomers, refugees, and undocumented immigrants
    • People with disabilities (physical, sensory, developmental)
    • Persons experiencing housing insecurity or homelessness
    • Individuals facing food insecurity
    • Survivors of violence, trauma, or abuse
    • People with substance-use disorders
    • Individuals with limited literacy (health, digital, or linguistic)
    • Rural or remote populations with limited access to services
    • Low-income and working-poor families
    • Persons without access to stable healthcare (e.g., uninsured)
    • Sex workers
    • Youth in care or those transitioning out of care
    • Single-parent households with constrained resources
    • People affected by systemic discrimination (e.g., anti-Black racism, colonialism, ableism)
    • Other community-specific or context-specific marginalized groups: Indigenous populations, Women, children, Teens, Incarceration, war-torn countries, etc…

Now that you have identified your chosen community, next examine the determinants of health that impact them the most.

    2. Social and Structural Determinants of Health (~1-2 paragraphs)

In this section analyze the key determinants of health shaping health outcomes and inequities in the selected population. Required Content

    • Identification of the most relevant (top 3) Canadian determinants of health affecting the community (select those that matter most, do not list all)
    • Explanation of how each determinant influences:

o Health status o Access to services o Inequities or unmet needs

    • Use of population-level data and scholarly/governmental sources Examples of determinants may include:
    • Income and social status
    • Housing and neighbourhood conditions
    • Employment and working conditions
    • Education and literacy
    • Food security
    • Social support and isolation
    • Access to health and social services
    • Culture, language, immigration status
    • Indigenous status and impacts of colonization
    • Systemic racism and discrimination

    Guiding Questions

    • Which determinants most strongly shape health in this community?
    • How do these determinants interact?
    • What experiences or impacts are not fully captured in existing data?

An examination of the determinants of health should provide insight into priority areas/concerns/ inequities that require or prompt the Community Assessment.

    3. Purpose and Focus of the Community Assessment (~1-2 paragraphs)

In this section explain why a community assessment is needed, based on preliminary evidence (ie. the determinants of health).

    Required Content

    • A clear purpose statement for the assessment
    • A preliminary issue or concern or inequity that has been identified, prompting the assessment
    • Population-level data supporting the need for assessment (e.g., census data, public health reports)

    Guiding Questions

    • What signals suggest a need to understand this community more deeply?
    • What do existing data suggest, but not yet explain?
    • What questions remain unanswered?

Next, identify how the community assessment will be conducted (Assessment Approach).

    4. Community Assessment Approach (~1-2 paragraphs)

In this section describe your approach to conducting the community assessment.

Although no primary data collection is undertaken, students are expected to demonstrate how a structured community assessment process was applied using available evidence. This is a theoretical Community Assessment.

    Required Content

    a. Assessment approach

    • Identify and describe the type(s) of community assessment strategies will be used (primary and secondary sources), such as: o Literature o Needs assessment o Asset and resource mapping o Priority population assessment o Community profile or environmental scan

    Guiding Questions

    • What type of assessment best fits the purpose?
    • Why is a multi-method approach beneficial?
    • How does this design support equity and inclusion?

This section is brief as you will be describing each data sources/collection approach (primary and secondary sources) separately the next sections.

Now, that you have identified how you will be collecting the data, next begin to populate the details of each and summarize your findings.

    5. Data Sources and Evidence Base (Secondary sources) (~1-2 pages)

In this section identify, justify and summarize the findings from existing data sources used to conduct the community assessment and demonstrate the ability to critically appraise population-level evidence. In this section only secondary data sources (academic and grey literature) will be discussed.

    Required Content

    1. Description of data sources

Identify multiple credible sources used in your assessment, such as:

  • Assignment 4.1: Preliminary Search for Evidence

You will now conduct a preliminary search for evidence regarding your approved topic. This assignment will help you learn how to conduct an effective database search for current literature about nursing practice. This is also the start of you finding articles for your Synthesis of Evidence Paper. Assignment Guidelines Complete the following steps: Identify your PICO(T) question. Perform a search in BU library databases-you will want to search multiple at one time. Please review the information created by our Librarian, Dr. Neilson-Ott found under Module 3 AND videos located under this module. In nursing we most often use Academic Search Complete, EBSCO, CINHAL, Health Source, PubMed, etc..- you will select 2 of these databases to conduct your search. Select keywords from your PICO(T) question with which to start your search. Identify subject headings for your search. Combine searches using the Boolean connector OR or AND. Limit the final search by selecting defining parameters (i.e., English, research, peer reviewed, 5 years, U.S. geographic, etc). Create a file with a table to display the results of your search. Please use the Sample Database Table for guidance. Create the table in a Word document using the Sample Database Table for PICO(T) Question (PDF) Download Sample Database Table for PICO(T) Question (PDF). You will have 2 tables for submission, one for each database Read the Professors Tips below for more guidance Professors Tips The articles must include only current evidence (published within the last 5 years). You may use class resources, but these will be in addition to the final 12 published works. Do not use secondary references unless absolutely necessary (obtain approval from the instructor before using secondary sources). Do not use literature reviews, systematic reviews, integrative reviews, or meta-synthesis unless approved by your professor as you should be synthesizing each study yourself instead of synthesizing what someone else synthesized. As we move forward, you will need to determine if you are able to obtain enough quality articles for this course. If you cannot find enough articles for this PICO(T) you should consider refining/editing your PICO(T); it is OK as long as you are still using the topic your instructor approved. Some of you may find that after doing this preliminary search for evidence, the direction of the PICO(T) question may change.

  • Electronic Health Records (EHR)

307 Module Five Activity Guidelines and Rubric Overview Nurses are sometimes tasked with gathering and presenting information to others. Nurses who are interested in the connections between technology and healthcare should be able to review a piece of technology for different qualities and characteristics. One of the things to consider when evaluating a type of technology is the regulating organizations and standards that may be important for implementing it into a healthcare system. Another consideration is any ethical connections between the technology and its use for an organization. Fortunately, nurses have the Code of Ethics for Nurses With Interpretive Statements to guide their practice, and it can be used to evaluate ethical concerns with the implementation of a healthcare technology. Concerns about privacy and confidentiality must be assessed in connection to any technology that will be used in healthcare. The electronic health record (EHR) is a technology that has been used in healthcare for a while, and therefore there is information that can be gathered about its use in healthcare from the perspective of regulations, ethics, and privacy and confidentiality. Scenario You are an active member of your organizations Informatics Council and a vocal advocate for the use of EHRs. The Informatics Council is responsible for educating all users on various aspects of the healthcare technologies that are used at your organization, including federal and state regulatory requirements. You have volunteered to create a PowerPoint presentation about regulations, ethical principles, and privacy concerns as they pertain to the EHR. This presentation will be disseminated to all healthcare providers in your organization. Directions Using the Module Five Activity Template in the What to Submit section, identify regulatory organizations related to the EHR as well as the relevant ethical principles and privacy concerns with this healthcare technology. To identify relevant ethical principles, use the ANA Code of Ethics for Nurses With Interpretive Statements, which is located in the Resources section of this module. It may be helpful to review the tutorial linked in the Supporting Materials section. Use at least two scholarly, current (within the last five years) references to support your work. Specifically, you must address the following rubric criteria: Organizations and Regulations Regulations and Standards: Discuss two organizations that provide regulations and standards associated with the EHR. Healthcare IT Efforts: Identify one professional organization that supports healthcare technology efforts. Use of the EHR: Discuss one law or regulation that directly impacts a use of the EHR. Ethical Principles ANA Code of Ethics: Describe one provision in the ANA Code of Ethics for Nurses With Interpretive Statements that aligns with the use of the EHR. Connection: Explain the connection between the provision and the use of the EHR. Privacy and Confidentiality Privacy: Explain one concern associated with privacy associated with the use of the EHR. Confidentiality: Describe one aspect of confidentiality that is a concern when using an EHR. What to Submit Submit your Module Five Activity Template with speaker notes submitted as an additional Word document. Submission should include 3 slides that follow APA Style standards. The title and reference pages are not included in the slide count. Sources should be cited according to APA style.
