  • Interview

    Brief Paper: At the start of the semester it is recommended that you make appointments to meet and interview a clinical social worker and a home health nurse for Module D activities (check dates on course schedule). The purpose of the interviews is to determine how best to prepare a patient for transition from acute care to home or rehabilitation facility and identify strategies to reduce hospital readmissions. Complete the learning activities for the module and compose the questions you want to ask your interviewees. The paper should be between 750 and 1000 (APA formatting, max 2.5 pages double-spaced) words not including title-page and references. Include the names and credentials of your interviewees. Some suggestions for questions are offered below.

    • The types of patients they care for and the assistance that is provided.
    • The level of engagement with caregivers and family members.
    • Stressors faced by caregivers.
    • What has been their professional observation about the knowledge patients have regarding disease, medication needs, post-hospital rehabilitation plans?
    • What are educational gaps on healthcare of the clients they see?
    • Ideas on how to improve discharge teaching for the patient and caregiver.

    For your paper (and suggested headings) you will need to include:

    1) Names and credentials of interviewees.

    2) Identify your interview questions,

    3) Summarize the findings of your interviews with insights you gained in-light of the material you studied in Module D.

    4) Identify strategies to reduce the incidence of hospital readmissions.

    Be sure to protect the privacy of any patients who might be identified during the interview.
