Brief Paper: At the start of the semester it is recommended that you make appointments to meet and interview a clinical social worker and a home health nurse for Module D activities (check dates on course schedule). The purpose of the interviews is to determine how best to prepare a patient for transition from acute care to home or rehabilitation facility and identify strategies to reduce hospital readmissions. Complete the learning activities for the module and compose the questions you want to ask your interviewees.
