For this assignment Perform a complete health history on a self-selected patient. For this assignment, you will need to identify a family member or friend as your selected patient for the entire assignment. No demographic or identifying information is to be shared for this assignment. Use chapter four of your Jarvis (2020) textbook as a guide to complete this paper. Synthesize your findings in a narrative, scholarly paper format based on the directions below. Structure and Content: No more than six pages in length, double-spaced, not including the title and reference pages. 12 point font Times New Roman or 11 point font Calibri. Title page in APA style. Complete sentences in narrative format. Sections of your Paper: Introduction (no heading): Briefly introduce your patient to the reader. Include biographic data, source of history, and present history or history of present illness. Past Health: Childhood illnesses, accidents or injuries, serious or chronic illnesses, hospitalizations, operations, obstetric history, immunizations, last examination date, allergies, and current medications. Culture and Genetics: Briefly highlight in the findings of the family history identified in the genogram discussion. Discuss any pertinent cultural issues. Review of Systems: Present a brief overview highlighting only abnormal findings. Functional Assessment: Use your Jarvis (2020) textbook as a guide. You may want to discuss self-esteem, self-concept, activity/exercise, nutrition/elimination, sleep/rest, spiritual resources, coping and stress management, personal habits, alcohol, illicit or street drugs, environmental/hazards, intimate partner violence, and occupational health. Perception of Health: Conclude with a summary of your patient’s perception of health. Identify their health concerns, goals, and expectations from their health care providers. General Tips Refer to Chapter 4 (Jarvis, 2020) for information on each required health history area. *Be sure to follow the Written Assignment Rubric. CHANGE IMPLEMENTATION AND MANAGEMENT PLAN
To Prepare:
- Review the Resources and identify one change that you believe is called for in your organization/workplace.
- This may be a change necessary to effectively address one or more of the issues you addressed in the Workplace Environment Assessment you submitted in Module 4. Change Implementation and Management Plan
Create a 5- or 6-slide narrated PowerPoint that presents a comprehensive plan to implement changes you propose.
Your narrated presentation should be 56 minutes in length.
Your Change Implementation and Management Plan should include the following:
- An executive summary of the issues that are currently affecting your organization/workplace (This can include the work you completed in your Workplace Environment Assessment previously submitted, if relevant.)
- A description of the change being proposed
- Justifications for the change, including why addressing it will have a positive impact on your organization/workplace
- Details about the type and scope of the proposed change
- Identification of the stakeholders impacted by the change
- Identification of a change management team (by title/role)
- A plan for communicating the change you propose
- A description of risk mitigation plans you would recommend to address the risks anticipated by the change you propose
Alternate Submission Method
You may also use Kaltura Personal Capture to record your narrated PowerPoint. This option will require you to create your PowerPoint slides first. Then, follow the Personal Capture instructions outlined on the
page. This page will walk you through downloading the tool and help you become familiar with the features of Personal Capture. When you are ready to begin recording, you may turn off the webcam option so that only “Screen” and “Audio” are enabled. Start your recording and then open your PowerPoint to slide show view. Once the recording is complete, follow the instructions found on the “Posting Your Video in the Classroom Guide” found on the Kaltura Media page for instructions on how to submit your video. For this option, in addition to submitting your video, you must also upload your PowerPoint file which must include your speaker notes.

Begin researching your topic.
- Explore the impact of the issue nationally and globally.
- Make an appointment with the WCU Librarian to discover supporting literature related to the practice issue, national and global impact, and policy addressing the practice issue.
- Address the impact or influence of professional associations in relation to the practice issue. Identify professional associations interest in the practice issue. Explore how and why professional associations influence the issue. What is the impact of this influence? How are national and global professional associations different than local and state associations?
- Find interviews from nursing leaders, policy makers, or politicians addressing the practice issue on a national or global level.
- Review the practice issue at the national and global level through contact with or review of national and global health organizations.
STEP 3. CREATE
Record your findings into an infographic to display your findings. Include the following:
- Identify and define the topic area and practice issue
- Description of the practice issue
- Impact to overall quality of patient care
- Impact to the nursing profession in general
- Identify relevant legislation and policies related to these issues and their effects on nursing practice
- Define the audience that the infographic will apply to. Keep in mind the audience is different than your previous assignment.
