Nursing Considerations:

• Analyze Mrs. Thompsons medical history and current health status. Is she at risk for acute and/or Chronic complications?
• Are there any potential gaps in her care or medication management that you think needs addressed?
• Risk Stratification:
• Using the clinical data provided and the AAFP Risk stratification tool, classify Mrs. Thompson into a risk category for case management (e.g., high risk, moderate risk, low risk AND Levels 1-6) and explain why you have placed her in that risk category. In your reading this week, the authors highlighted the importance of identifying various perspectives and worldviews regarding the care and management of patients across the lifespan. The resource articles provide illustrations of how a practice environment analysis has been applied in tertiary and ambulatory care settings.

Using the 7 domains of analysis noted in Figure 2 of the Pursio et al. (2024) article and the TOP10 items (Extracted of PES-NWI) from the right-hand side of Table 5 of the Martinez-Riera et al. (2020) article develop a professional presentation that aligns with the requirements noted below.
