  • PICOT

    Im RN in PCU.

    1. Choose a topic you genuinely encounter or anticipate (e.g., CAUTI reduction, nurse-led education for HF readmissions, hourly rounding and falls, early mobility in ICU).
    2. State the problem clearly in one sentence (who/where/what impact).
    3. Draft each PICOT element (use the template):
    • P Population/Problem: age, condition, setting (be specific).
    • I Intervention/Exposure: what you will implement (education, protocol, bundle, technology).
    • C Comparison: usual care, another intervention, or no intervention.
    • O Outcome: measurable and clinically meaningful (rates, scores, days, %).
    • T Time: realistic window (e.g., during hospitalization, 30 days, 3 months).
    1. Assemble into one sentence(therapy/etiology/diagnosis/prognosis/meaningpick the type that fits).
    2. Create your search plan: list keywords + synonyms for each PICOT element; pick databases. Ex: CINAHL/PubMed/Proquest/Joanna Briggs
    • (nurse-led OR nursing) AND (sepsis OR delirium) AND (bundle OR protocol) AND (outcomes OR readmission)
    • (handoff OR handover) AND (I-PASS OR SBAR) AND (errors OR safety) AND nursing
    1. Check feasibility: Is the population accessible? Is the outcome measurable? Is the time frame realistic?
    2. Polish for clarity: avoid jargon, keep one main outcome, and ensure each element is explicit.

    Example 1 Falls (Therapy)

    • P: Adults 65 on a med-surg unit
    • I: Structured hourly rounding protocol
    • C: Usual care (no structured rounding)
    • O: Inpatient fall rate per 1,000 patient-days
    • T: Over 8 weeks
    • PICOT: In adults 65 on a medical-surgical unit, does a structured hourly rounding protocol, compared with usual care, reduce the fall rate per 1,000 patient-days over 8 weeks?

    Common Pitfalls (and Fixes)

    • Too broad population add age, unit, or diagnosis.
    • Vague outcomes (better satisfaction) specify tool/metric (e.g., HCAHPS domain score).
    • Missing comparison use usual care if no alternative is planned.
    • No time frame tie to an encounter or follow-up period.

    Quick Quality Checklist

    • Population is specific (age/setting/diagnosis).
    • Intervention is actionable and describable.
    • Comparison is appropriate (usual care/alternative).
    • Outcome is measurable (include a metric).
    • Time is realistic.
    • One primary outcome (avoid stacking multiple).
    • Words are searchable (you have synonyms).

  • Assignment 9

    Health Literacy

People need basic health literacy skills to make good decisions about their health care. However, 9 in 10 adults do not have the basic skills they need to manage their health. Patients with low health literacy may be unable to follow directions in adopting healthy lifestyle behaviors, participate actively in their care, self-manage their medication regimens, and follow treatment recommendations of health care team members. Patients with low health literacy face numerous challenges in maneuvering through the health care system and self-managing chronic illnesses.

Living with a chronic gastrointestinal (GI) disorder, such as inflammatory bowel disease, can be very challenging for patients. A range of skills can be required to deal with some of the issues and uncertainties associated with living with a GI disease.

  • Discussion Week 11

    Instructions:

This week you are learning quite a bit regarding the older adults’ experience with cardiovascular, pulmonary and neurological problems. According to the CDC, 60% of older adults are living with two or more chronic conditions. This starts to become a challenge as you manage and educate multiple conditions and let’s not forget the polypharmacy involved (NIA, n.d.).

    Initial Post Requirements:

Please critically evaluate the subjective and objective information provided to you in the attached file below, and use the SOAP template as a reference.

1? The first part of the discussion board is to identify all pertinent positive and, negative information and list missing information. Then create a differential diagnosis list with at least 3 possibly actual diagnosis based on your findings. Here are the headings you should use:

    • Pertinent positives
    • Pertinent negatives
    • Missing information
    • Differential diagnoses

2? The second part is to create a plan utilizing clinical practice guidelines for the priority diagnosis. Here are the headings you should use:

    • Diagnostics
    • Therapeutics

Where is the largest growth of men in nursing? Why? Compare and contrast the current demographics of RNs, nurse educators and the US population. What changes need to be made to even out these demographics? Per the Rubric, you are required to use Scholarly article(s) to answer the question, such as journal articles. The textbook is NOT a “scholarly article.” Sources used must be cited and referenced per APA guidelines. You must also submit ONE document that includes a screenshot of the FIRST PAGE of EACH SOURCE USED. Sources listed without a corresponding screenshot will earn a zero in that part of the Rubric
  • Medication error root cause analysis

For over 30 years, Institute for Safe Medicine Practices (ISMP)[ has been a global leader in patient safety as the first non-profit organization dedicated to the collaborative development, education, and advocacy of safe medication practices. Nursing students may recognize publications from ISMP throughout their clinical areas. Often, ISMP safety recommendations are utilized in equipment purchasing and policy development. Information linked at the ISMP site will assist you in this assignment. The ISMP Assess-ERRTM Medication System Worksheet is a quality improvement tool There are 2 parts to this written assignment- (a) completing the ISMP worksheet and (b) a paper describing the possible root causes to this error. Part 1- review of ISMP med error worksheet with application to a case study Review and download the ISMP Assess Medication Error Worksheet. Download ISMP Assess Medication Error Worksheet. It is designed to help with error report investigations. Read and study the details of the medication error in this ISMP case study article. Download ISMP case study article. As you read the article, highlight critical information. Part 2- written analysis Students will address at least five of the possible causes (left column) outlined in the ISMP Worksheet Each cause should be identified in a separate heading within the paper. Once completed, use the worksheet as a template to describe in paragraph form/APA 7th edition student paper format, the possible causes that contributed to the negative outcomes in this case study. For each cause, write a paragraph(s)/APA 7th edition format explaining: What was/were the possible causes/contributing factor(s)? Why did you identify this as a possible cause(s)/contributing factor(s)? What are possible solutions to prevent further errors from occurring? Look from a systems-level/perspective. Validate your solutions through the use of a total of five evidence-based practice research sources (e.g. Think of a variety of solutions which may include education, policy change, equipment, remediation, etc. Be sure to think of hospital/facility/role level solutions, not individual-based solutions (i.e. hospital policy rather than individual RN practice) How and why do you think differently now? How will this new thinking impact you as a future nurse? Include your Assess-ERR worksheet when you submit your final paper of a minimum of five pages or a maximum of seven pages; do not count your cover page or reference page as part of the total length. Part 1 is completed already; and would you be able to complete this in and hour instead (11:00 pm).
