    The maternal mortality rate for Black women in Red Oak County is notably high. Black women experience 47.5 deaths per 100,000 live births, compared to 16.8 for White women. To further complicate things, only 30% of Black women receive any kind of prenatal care, compared to 85% of White women. The local health department has been trying to determine why that inequity exists but does not yet have the budget to conduct a survey or focus groups.

    Many women in Red Oak County face a range of health issues before and during pregnancy, including hypertension, diabetes, and complications from preterm births. The prevalence of these conditions is higher in the most economically disadvantaged areas, where access to preventive care and early intervention is limited. For example, the rate of low-birth-weight infants is significantly higher among Black women (14%) compared to White women (7%). A significant number of pregnant women are engaging in behaviors such as smoking, unhealthy eating, and lack of exercise. About 25% of pregnant Black women in the county smoke, as compared to 10% in higher-income areas. Traditional health practices and a general historical mistrust of the health care system contribute to the underutilization of available services, particularly among the Black population.

    A recent community health assessment highlighted the urgent need for comprehensive maternal health interventions and addressing social determinants of health such as income, education, and housing. The assessment revealed that many Black women delay or miss prenatal care appointments due to transportation, financial barriers, distrust of the medical community, and language and cultural barriers. Additionally, the high crime rates and unsafe living conditions in certain neighborhoods also limit safe mobility and access to the needed services, creating an environment of chronic stress that further impacts maternal health. The rate of maternal mental health issues such as postpartum depression is three times higher in Red Oaks Black community than in Red Oaks Hispanic community and two times higher in Red Oaks Black community than in neighboring, higher-income White communities.
