  • Case study analysis

NUR3125 Pathophysiology: Case Study Analysis Instructions The purpose of this assignment is for you to analyze patient data and synthesize their condition/disease with what you are learning in this course. Read and follow the instructions carefully: Analyze actual data from a client for whom you have provided care. Please carefully pay attention to HIPAA guidelines, removing all names, dates, medical record numbers, room numbers, etc. from your paperwork to protect client confidentiality. You may use initials or a pseudonym such as John Doe. You can be creative with the name. In a previous term, I had about 16 John Does. Also, do not name where this happened- such as the name of the hospital or facility. For example, you may state a hospital or facility within northeast Florida. Alternately, if you are not able to gather such information, you may create your own scenario and case study. You are looking at data on a real patient, this case analysis is not set up as a report. Prepare a paper 6-8 pages (not counting the title page and the references) using formal APA format. Include a minimum of four references. One source may be your text and a minimum of one English peer-reviewed nursing journal article (less than 5 years). Articles from non-nursing peer-reviewed journals, including those published outside the United States, and websites ending in .gov, .org and .edu may be used as a third resource if published within the last five years. Sources such as WebMd are not acceptable. Save your paper as a Word document and name it: lastnamefirstinitial Case Study. For example, if I were to submit my paper it would be CorsoM Case Study Include an Introduction, Discussion, and Conclusion sections. After a brief introduction, create Level One headings for each of the following: Patient Background: (include social history, allergies, current medications, and any labs or diagnostic tests related to the patients condition). For related labs or vital signs, you may create a table within your paper. Make sure to highlight abnormal findings (with a bold H or L). Pathophysiology: this section must be related to the condition your patient displayed during the time of your care. You may also include pre-existing conditions that correspond to their current state. A good example of what patho to include… If your patient has diabetes, but is in the hospital or seeking care due to a respiratory issue, do the patho for the respiratory issue, not diabetes. Be descriptive in your explanation of the pathophysiology of the disease/condition. Do not give a brief overview of the pathophysiology. This section should include what is happening at the cellular level within the body for that specific disease process or disorder. You may use your textbook. Include an original chart, diagram, algorithm (developed by yourself) that displays the pathological process of the patients condition in this section. If you use someone else’s works, you will need to credit the source- for example, you will have to place a caption under your chart/diagram stating Fig. 1: adapted from….. Failure to cite properly will lead to points deducted from your grade! Treatment Plan: this section needs to include medical treatment (prescribed by the MD) and nursing treatment/care. Include priority concerns. Worsening: what would happen if your patient’s condition worsened? If there was no worsening of conditions, what worsening conditions could have occurred? (When writing about what-if symptoms got better or worse, there are a few things to consider. If your patient improved, you could write about that, but you may need to go further. For example, a patient with an amputated leg may improve to the point of healing but not be able to wear a prosthesis due to pain. But what if…it is possible this patient could improve to the point of being pain-free and being able to wear one. If your patient did not improve, do not leave it blank. Write what would have happened if he had improved. The same applies to getting worse). You are expected to explain what happens if the patient improves or worsens, regardless of what actually occurred. Improved Conditions: What would happen if your patient’s condition improved? If it did not improve, what improvement could there have been? You are expected to explain what happens if the patient improves or worsens, regardless of what actually occurred. Conclusion: Summarize your findings So, the sections in the paper would look something like this: Your introduction paragraph(s). Discussion Patient Background Your paragraph(s) to include a summary of the pertinent patient background Pathophysiology Your paragraph(s) to explain the patho regarding the disease process or disorder on a cellular level. Include diagram or chart Treatment Plan Your paragraph(s) regarding the prescribed treatment plan of the physician AND the nursing treatment Worsening Conditions Your paragraph(s) to explain what happened or what might happen if the condition became worse Improved Conditions Your paragraph(s) to explain what happened or what might happen if the condition resolved or became better Conclusion Your paragraph(s) Submission of Assignment: Submit your assignment into the proper link within Canvas (Listed as Case Study Analysis). Your paper will automatically be scanned by Turnitin (you may include your title and reference page in your submission- I can re-scan it on my side). Your similarity index must be <20%. You can submit your paper up to 3 times in Turnitin before the due date. Do not worry if your index is high because of your reference and title page, it is ok, I can go back and re-scan your paper!! However, if your paper shows a high similarity index on certain sections and you did not attempt to revise this (or properly cite your sources), then worry! Please follow these instructions carefully and view the exemplars within Canvas that are available for your viewing! The examples are from previous assignments so some items may be different. The papers will give you an idea of the basic layout of the paper. They will also give you ideas for the original chart, diagram, algorithm (developed by yourself) that displays the pathological process of the patients condition.

  • Discussion board 4

Respond to the questions posed in the weekly discussion board. This is an opportunity to share your knowledge with your peers, think of different considerations, bounce ideas off of each other… and continue to develop your clinical judgment skills!

    1. A nurse is performing an examination of the oral cavity and throat for a client who complains of a sore throat. The nurse depresses the tongue with the tongue depressor to visualize the pharynx, but the client gags, making the examination difficult. How should the nurse conduct the examination?
    2. A nurse is examining the internal nose of a client through an otoscope. The client refuses to undergo the otoscopic examination because the otoscope hurts the clients nose. How should the nurse deal with the situation to foster client compliance with the examination?
    3. A nurse examines the sinuses of an elderly client. The nurse palpates the frontal and maxillary sinuses. The nurse finds the presence of crepitus and documents the finding as presence of exudate in the sinus. Is the documented finding correct? How should the nurse ensure an accurate assessment of the sinus?
