    Assignment Overview

    Social determinants of health (SDOH) significantly influence health outcomes, access to care, and health equity. During your practicum experience, you will identify, observe, and address SDOH affecting individuals, families, or populations served at your clinical site. This assignment emphasizes real-world application of population health concepts within community settings. Each student will complete a PowerPoint that addresses all of the components listed below. Please make sure to include at least 2 references with this assignment, follow APA format, include practicum site name with city, and maintain confidentiality per HIPAA and FSU policy.

    Purpose

    This practicum assignment allows students to:

    • Apply SDOH concepts directly to community-based nursing practice
    • Analyze how social, economic, and environmental factors affect health
    • Identify the nurses role in mitigating barriers to care
    • Connect practicum experiences to population health outcomes

    Assignment Components

    1. Identification of SDOH

    During your practicum hours, identify two (2) social determinants of health that significantly impact clients or the population served at your site. Examples include:
