    Applying the Transtheoretical Model of Behavior Change (TTM)

    The purpose of this paper is to interview a volunteer patient in order to identify behaviors that may affect your patients health and well-being. In order for your patient to affect a healthy change, you will apply Prochaska & DiClementes Transtheoretical Model of Behavior Change (TTM) to your volunteer patient as a health promotion activity. The goal is to identify the stage of the TTM the patient is in and design an evidence-based nursing intervention to assist the patient in changing this behavior, which will allow them to the next stage of the Transtheoretical Model.

    Instructions and Requirements:

    o Interview Your Patient and identify a behavior that needs to be changed. For example, while obtaining your patients health history you may discover that the patient is a smoker, does not get enough sleep, has poor eating habits, is a diabetic and does not monitor blood sugar or they have a sedentary lifestyle. These behaviors would impact your patients health.

    o Write your paper following the outline found on the next page

    o Use APA Language which is clear, concise, bias free and objective. Choose precise strong words, eliminate unneeded words, and use grammar properly. Do NOT use direct quotes. Paraphrase information, which means rephrasing the words of the author and putting their thoughts in your own words.

    o APA Formatting: Paper should be correctly formatted in APA 7. See APA handout

    o Title page: Include properly formatted a title page with all the required information

    o Basic formatting:

    1) Double-space document,

    2) use Times New Roman size 12 font,

    3) justify text to the left

    4) indent the first line of each paragraph .5,

    5) place page numbers in upper right corner, and

    6) add title to the first main body page only, centered in bold

    o Headings: Include all required APA level headings as indicated in outline

    o Intext Citations: Cite information from the sources within the body of your paper.

    o Reference Page: Include a reference page with all the references cited in the paper

    o Paper Length: The maximum paper length is 5 pages. Any text over 5 pages will not be considered for grading.

    o Grading: Please review rubric on Blackboard to best meet requirements.

    o Submit: Upload the assignment to the Turnitin link by the deadline on your syllabus. Do not submit your paper with your name including the title page when you upload it to Turnitin.

    Application of TTM for Behavior Change Outline Part I-Introduction (Do not use this heading)

    1. Identify the purpose of the paper.

    2. Introduce the patient (according to HIPAAdo not use the patients full name or relation to you. Use the initials or the first name only).

    3. The behavior to be changed: Remember to identify a behavior and not a symptom or condition. (For example, with the condition of obesity, the related behavior might be eating large portions, poor food choices, lack of planning, lack of exercise, etc. However, weight loss is not a behavior; it is a symptom. Select ONE behavior to focus on though you may need to identify that other, related behaviors exist that contribute to the condition).

    4. Introduce the TTM: Include what is it, who created it, what is it use for (CITE)

    5. Explain the expected outcome from the behavior change a measurable objective.

    Part II-Health History

    1. Summarize your patients health history. You should not repeat the entire patient history, but rather analyze the history and discuss only the relevant facts from the patient and family histories that pertain to the behavior you are addressing

    2. Identify at least two actual or potential health risks.

    Part III-Rational

    1. Using a minimum of 3 peer-reviewed references that are no more than 5 years old, provide the rationale for the chosen behavioral health risk that your patient needs to change to promote health in his/her life

    2. Why does the patient need to change the behavior? What is the science that explains the health risks?

    3. Provide current, scientific evidence from the peer-reviewed literature as to why the risk factor is important to address in relation to health promotion

    Part IV Overview of the TTM: Explain the theory behind the model

    A. Stages of TTM: Review the stages of TTM. Articles included in the course sessions may be used to write this portion of the paper. However, you may NOT use the textbook from the course for this section of the paper. This is a brief discussion of the 5 stages based on their temporal dimensions. (Cite this section using articles provided by the instructor)

    B. Outcome Measures and Processes of Change:

    1. Identify and explain the three outcome measures. State how a patient looks at the pros and cons of changing a behavior by combining the processes with the outcome measures.

    2. Identify and briefly explain the ten processes of change

    Part V-TTM in Practice: Describe how TTM is used in practice as it relates to your patient.

    1. Relate the stages, processes of change, and decision-making aspects of TTM (decisional balance/temptation/self-efficacy) to your patient.

    2. Identify which stage your patient is in and which of the processes and outcomes are operating in your patients health belief system.

    3. Identify one appropriate nursing intervention that may assist your patient to progress to the next stage of the Transtheoretical Model while improving their health outcomes. Support your intervention with evidence with at least 2 articles from the peer-reviewed literature (e.g., identify how applying one of the processes of change may motivate the patient to move to the next stage, or identify at least one barrier that may prevent this progression).

    4. State what nurses role would be in guiding/coaching the patient through this intervention (remember, in the TTM the patient who makes the change, not the nurse).

    5. Be sure to consider culturally competent care when designing your interventions. Identify at least one barrier that may be affecting your patients progress.

    Part VI-Conclusion (Do not keep this heading)

    1. This should be a conclusion of your paper, not a conclusion of your patient (the conclusion about the patient should be in the previous section).

    2. The conclusion of the paper re-iterates what was discussed in the paper and its relevance to nursing practice.

    It should also tie-in the items discussed in the introduction

    Understanding Trauma Across Professional Roles in Sports and Education Video submission in content in D2l

    Objective:

    Graduate students will analyze traumas impact on individuals, teams, and organizations by reflecting on a professionals prerecorded video presentation. Students will present their reflections as a video. They will also engage with peers responses to expand their understanding of trauma-informed practices across different roles, such as teachers, coaches, athletic directors (ADs), strength trainers, and sports psychologists.

    The video is not specifically directed toward our field, but we must understand what our students and athletes are going through to be able to understand their behaviors. In the video he talks about the same trauma in two different people. What could make it different is if they had the ability or tools to cope with what happened in their life.

    Part 1: Analyze the Professional Presentation

    1. Watch the Prerecorded Video:

    The video features a professional discussing trauma recognition, its effects, and trauma-informed strategies you can use as one of the following:

    o Teachers

    o Coaches

    o Athletic Directors (ADs)

    o Strength Trainers

    o Sports Psychologists

    2. Initial Response Options:

    After watching the videos, students will reflect on the content and create a response in the following formats:

    Video Post (5 minutes) minimum: Record a video summarizing your insights.

    Your response should include:

    Key Takeaways: Highlight three major points from the video that resonate with you.

    Connection to Your Role: Reflect on how these insights apply to your current or future career (e.g., as a coach, AD, teacher, etc.).

    Practical Application: Share one trauma-informed strategy you could adopt or recommend based on the content.

    For this assignment, I will research one vegetable (or fruit) and one meat to determine the energy balance of each food. The goal is to find the amount of energy contained in the food and compare it to the amount of energy required to produce the food.

    For each food, I will locate sources that provide the following information:

    1. The number of Calories in one pound of the food.
    2. The amount of energy required to produce one pound of the food (or another unit that can be converted to pounds).

    The sources must be credible and clearly cited. If a Wikipedia page is used to find information, the original source cited by Wikipedia must be used instead.

    For each food, I will submit:

    The energy required to produce the food (with units such as BTU, MJ, or Calories)
