  • M4.1 High Protein and Vegetarian Diets

    Part I:

In this discussion, we will assess our individual protein requirements and analyze the benefits and risks of various popular high-protein diets.

First, lets start with a question based on your opinion and on what we have learned so far in past modules:

    1. In your opinion: Figuring out how much protein is enough can be very confusing for many. We can find any number of articles or websites that suggest that a high protein diet is good for us, and just as many other sources arguing it is dangerous. Based on what we have learned so far this term, what are the challenges you foresee in adhering to a restrictive diet? Are there consequences to limiting your intake of certain foods?

    Evaluate

    Part II:

Now applying what youve learned from the readings and videos and your one-day food diary, lets go through the rest of the discussion together, one question at a time.

    1. Many people believe that they do not get enough protein. However most Americans consume more than their bodies need. Based on what you learned by tracking your typical one-day protein intake, how well do you think you meet your RDA for protein? Are there changes you would make? What would be your biggest challenge in changing your protein intake?
    2. High-protein diets continue to gain popularity, particularly for weight loss. Choose one popular high protein diet of interesteither from the readings or something in the popular press (e.g., Atkins, Paleo, South Beach, Zone, etc.). What are the health benefits to this diet? What might be some health risks or contraindications? Based on what we have learned, how safe is this diet to follow on a long-term basis?
    3. Now that we have discussed high-protein diets, lets consider the growing trends toward vegetarianism. Based on this weeks readings on vegetarian diets, what are some of the key health benefits and risks to following one of these plant-based diets? What challenges might a person face in changing their current diet to this type of vegetarian diet?

Assignment #1 – Special Considerations for Patient Populations Paper:

Construct a paper concerning one of the patient cases listed below. Please choose the patient case that interests you, one will NOT be assigned. (hopefully the class is starting to see a pattern here!). In your paper include the following:

Determine which medication would be most appropriate to prescribe from the assigned vulnerable population patient case medication choices.

Explain your rationale for choosing this medication. Explain why the other medications listed are not appropriate for this patient.

Explain the dosing schedule for the specific patient including the therapeutic endpoint.

Provide necessary education to the patient to review risks, benefits, and potential side effects of the medication.

Describe any necessary labs or additional diagnostics needed prior to prescribing this medication.

Explain how you might monitor efficacy or side effects of the medication.

Include any additional collaboration or education to others that would be necessary for this patient. Consider family members, home health care, primary care providers, etc.

What would you need to include in your assessment for a patient who may become nonadherent with your prescribing plan for your scenario? What are alternative treatment solutions based upon how you assessed? Describe your new treatment plan.

This Assignment requires a minimum of five (5) peer-reviewed, evidence-based scholarly references outside of course resources.

Note: You will need to include the APA style formatting.

Reminder: The College of Nursing requires that all papers submitted include a title page, introduction, summary, and references. The

provides an example of those required elements. All papers submitted must use this formatting.

    Due by Day

Special Considerations for Patient Populations Cases:

Patient 1: 82-year-old male presenting with cognitive decline. He had always been a very active individual. Over the past year, he has become less active in the community. Previously, he played cribbage weekly with friends at the local senior center but has been struggling with the math involved with the game and no longer attends the weekly card game. He had also volunteered as a crossing guard for the local public school. He quit doing that after he took the wrong turn to get home and drove around the town feeling lost. He acknowledges he is sensitive to this and can be irritable towards his children when they bring up these concerns. He was diagnosed with mild neurocognitive disorder after completing neuropsychological testing. Of the following medications, which would be the most appropriate to prescribe? Explain why you chose this medication. What would be the dosing schedule for this patient? Provide education to the patient and review risks, benefits, and potential side effects of the medication. In addition, explain why the other medications listed are not appropriate for this patient. Med List: lorazepam, olanzapine, memantine

Patient 2: 76-year-old female who recently moved from her home to an assisted living. After 1 week of moving in, she has become extremely confused. Staff have attempted to re-orient her, and she struggles to focus on what they are saying. She is only oriented to self. She has become very agitated and has even thrown breakable objects in her room. Her family is concerned stating she is out of it. The PCP ordered labs and UA. Labs are unremarkable outside of mild leukocytosis and positive dipstick analysis for nitrite and red blood cells. Med List: valproate, alprazolam, trimethoprim

Patient 3: A 33-year-old female that is 6 weeks postpartum. She presents with depressed mood, sadness, easily emotional, difficulties with sleep even when the infant is sleeping. She has lost weight and has a poor appetite. You note psychomotor retardation. She has limited interests in hobbies or bonding with the infant. Her partner is concerned and reports she is typically active and bubbly and enjoys the outdoors. She is diagnosed with major depressive disorder, with postpartum onset. She denies suicidal ideation, homicidal ideation, or infanticidal ideation. No psychotic symptoms are present. She is nursing the infant and wants to continue this hoping it will assist with bonding with the infant. Med List: lithium, paroxetine, sertraline

Patient 4: 10-year-old male who comes in with his father. He has been diagnosed with generalized anxiety disorder. He has constant worries that he cannot control. He feels nervous and tense. He is easily irritable. He struggles to settle down or relax. He is also quite fidgety and restless. He and his father are interested in medications to manage these symptoms. Med List: quetiapine, duloxetine, fluoxetine

Patient 5: 27-year-old male with a diagnosis of schizophrenia. He has been hospitalized three (3) times in the past 1 year due to symptoms. When symptomatic, he becomes disorganized and paranoid. He will respond to auditory hallucinations. He will scream out at night after seeing a shadow in his room. He has delusions that the CIA is poisoning food and will not eat for multiple days. He has unintentionally hurt his mother after slapping food out of her hand, as he did not want her to ingest the poisoned food. Symptoms are treated very well with paliperidone, but he struggles to remember to take it. Med List: paliperidone palmitate, clozapine, lamotrigine

    Assignment #2 – Writing Prescriptions

Create prescriptions using the patient information, medications, and indications listed below. Your goal is to demonstrate the accurate way to write a prescription that a patient would present to a pharmacy. The prescription should be written and signed. You will develop your mock prescriptions in a word document to include everything one might find on a printed prescription pad. You will write one prescription per page.

To write your prescriptions, please use the following patient information:

    Patient Name: Jane Doe

    DOB 1/1/2001

    Duration: 3 months

Write a prescription for each of the following medications:

    Sertraline, 100mg each day (depression)

Lorazepam, 1mg twice each day, if needed (panic attacks)

    Aripiprazole, 2mg before bedtime (depression)

Methylphenidate ER, 54mg every morning before school (ADHD)

Please do not submit a narrative about how these will be written. We want to see 1 prescription per page in a format that would be transmitted to a pharmacy!

  • Psychological disorders and mental health advocacy

Competency In this project, you will demonstrate your mastery of the following competency: PSY-215: Describe various diagnoses of psychological disorders Scenario At this point, Jamal feels enlightened (and empowered!) by how much he has learned about mental health during his time off between terms. He has learned about deinstitutionalization and its consequences for mental health and the public. Likewise, Jamal acknowledges how perspectives have changed regarding the duty of clinical professionals to provide treatment to those in need, as well as how social and cultural changes have influenced our conception of psychological disorders. In many cases, advancements in symptom classification and diagnostic assessment have caused us to question whether what we once viewed as disorders are actually disorders at all. Conversely, we now recognize new disorders that did not exist before. Given his newfound interest in mental health advocacy, Jamal wants to be able to speak to the many changes that have occurred within the mental health paradigm over time and how those changes continue to forge progressive new pathways in mental health reform. Jamal has recently created a blog to highlight the many ways in which we can change the conversation about atypical or culturally unexpected behavior. For this project, you will first create a blog post that compares the diagnostic features and underlying biopsychosocial factors of various psychological disorders. Your post will outline the history of mental health disorders, the impact of deinstitutionalization, and the shared symptoms across various diagnoses. In addition, your post will provide perspectives, considerations, and awareness in an advocacy message that helps to change the conversation about a specific disorder. Directions Part One After reviewing the scenario, use the Project One Template linked in the Supporting Materials section to complete your blog post. You will first review the various psychological disorders that youve learned about in the course and select three disorders from the list provided to focus on for this assignment. Next, in a minimum of 3 to 5 sentences per bullet for each psychological disorder, respond to each of the rubric criteria below to structure your blog post. Support your answers with credible sources when appropriate. In your own words, identify and describe the main diagnostic features of each of the three disorders selected. Consider biomedical factors: Describe what it means to characterize the three disorders as diseases of the nervous system and the body. Consider clinical factors: Describe the similarities between the selected. In what ways might these similarities impact both the diagnostic and treatment processes? Consider historical factors: Describe the ways the deinstitutionalization movement impacted patient rights, levels of care, and access to treatment for the three disorders you selected. Consider sociocultural factors: Describe the social and cultural changes that have caused shifts in the publics perception of the three disorders over time. Part Two For Part Two, choose one disorder from the three you discussed in Part One. Respond to each of the following prompts in a minimum of 3 to 5 sentences. Support your answers with credible sources when appropriate and address the rubric criteria. Consider the mind-body connection: Describe whether a discernible bidirectional relationship exists between the disorder and physical illness. Consider the impact of language and messaging: Describe how the words used (e.g., in casual conversation, in media communications) when discussing a disorder influence societal willingness or ability to manage the disorder. Consider the effect of lifestyle choices: Describe the small, but significant, changes in attitude/behavior a person can make that could help them manage the symptoms of the disorder.

  • Psychological Evaluation

The assignment has to be about a patient who has suffered from amnesia after a car accident and has to complete a psychological evaluation.

  • EHR Adoptions and Benefits

Develop a 2-3 page review of an EHR in which you identify EHR benefits, limitations, challenges, and applications. You will combine your experiences in the EHR Go activities with information from a case study.

    Expand AllIntroduction

In this course, you will have the opportunity to get your hands on an electronic health record (EHR)! The EHR is a systema group of elements working together to perform a function. HIM professionals need to be subject matter experts on the EHR’s various parts and how they are connected (Amatayakul, 2017).

Additionally, you will use your knowledge of the different systems to identify benefits, limitations, and challenges to using EHR. The textbook readings provide many benefits and hints at the limitations and challenges. Use your work in the EHR to help identify benefits and challenges.

Finally, you will compare how the benefits and challenges balance against each another. This involves the setting of goals for EHR use and strategic planning for the implementation of an EHR. Strategic planning includes setting priorities, such as deciding which applications are needed versus only desired. And, what should be the sequence of application implementation? The EHR Go activity will help you identify when applications are needed and which are nice to have.
