  • Part 2 of professional practice report

    As you have noticed, we have presented a lot of information in your studies thus far. Some of the information is similar and you may have found some conflicting information. The point of providing so many resources is to look at the same content from different perspectives. That is what we do with the literature, too.

    How does understanding and engaging stakeholders influence the implementation and sustainability of your intervention? How might this knowledge shape your approach to collaborative healthcare in your future practice?

    Note: The assessments in this course must be completed in the order presented; subsequent assessments should be built on both your earlier work and your instructors feedback on earlier assessments. If you choose to submit assessments prematurely, without considering and integrating your instructors feedback, your assessment may be returned ungraded, resulting in your loss of an assessment attempt.

    If you choose to make revisions based on feedback from a previous attempt, you should highlight your revisions in yellow. For example, if you made revisions from attempt one and would like the instructor to review the content when grading attempt two, the content must be highlighted. Therefore, the instructor reviews only the content highlighted on attempts two and three. If the entire paper is highlighted, the paper will be returned ungraded and count as an attempt. Track changes are not a substitution for highlighted text.

    Doctor of Nursing Practice-prepared nurses are well-versed in preparing written material and synthesizing content. In the Professional Practice Plan, you will draw from the information from the Professional Practice Report by creating an introduction. Then, you will describe the population, the use of stakeholders, and a problem question.

    For this assessment, write a 35 page plan demonstrating the skills of summary and synthesis in the development of an introduction, description of both the population and the stakeholders, a problem question, and a conclusion. Review the scoring guide to understand the assignment’s requirements, especially regarding headings and page length.

    • Write an introduction by synthesizing the context of Section II from the Professional Practice Report assessment.
      • Provide a summary using the MEAL plan for the practice site and problem. Revisit as needed.
        • Much of the information will come from your Professional Practice Report.
          • Make sure to address instructor feedback, if any, from the previous assessment and incorporate the feedback into your introduction.
        • Include your concise problem statement.
          • Remember that a good problem statement is one to two sentences and includes both the problem and a measurable objective.
        • Include a thesis statement that addresses the content from the Professional Practice Plan.
      • Describe the population at the practice site in terms of role, age range, diagnosis, procedure, etc.
        • The length of this section should be one paragraph.
        • In the description, be as specific as possible and provide details to convince the reader who your population is.
        • Consider the following when describing the practice problem:
          • What collective factors would include or exclude participation?
          • Is there both a primary and secondary population?
      • Describe the stakeholders at the practice site and their roles in the practice problem.
        • The length of this section should be two to three paragraphs.
        • Consider the following when describing the stakeholders:
          • Describe the stakeholders at the proposed practice site.
          • Explain plans to tailor your message to stakeholder groups.
          • Describe your strategy for obtaining cooperation and collaboration at your proposed site.
      • Formulate a problem question using the PICO format. Provide the details of each component of your PICO question.
        • The length of this section should be one paragraph.
        • Remember the PICO is a question; therefore, you should have appropriate punctuation.
        • Make sure to label the components of your PICO question.
      • Provide a conclusion synthesizing the concepts described in the Professional Practice Plan in a well-formatted paragraph.
        • Make sure not to include any new information in the conclusion.

    APA Style and Scholarly Writing

    • Convey purpose in a well-organized text, incorporating appropriate evidence and tone in grammatically sound sentences.
      • Write with a specific purpose in mind.
      • Adhere to scholarly and disciplinary writing standards.
    • Apply APA style and formatting to scholarly writing.
      • Include APA citations and references for a minimum of four scholarly references.
      • Your assessment should also meet the following requirements:

        • Length: Your report should be 35 pages long, excluding the title page, references page, and appendices. Be sure also to submit a PDF of the scholarly source you chose to summarize.

    Using APA (American Psychological Association) format is an essential part of academic writing. APA style helps writers clearly communicate ideas, properly credit sources, and maintain academic integrity. In this course, APA format is required for discussion posts, written assignments, and reference pages.

    Why APA Format Matters

    APA format serves several important purposes in academic work. First, it helps prevent plagiarism by clearly identifying which ideas come from outside sources. Second, it allows readers to easily locate and verify the sources used in your work. Finally, learning APA style prepares you for future coursework and professional environments where clear, ethical, and standardized writing is expected.

    In-Text Citations: How They Work

    In APA format, in-text citations appear within the sentence where a source is used. A citation should be included every time you paraphrase, summarize, or quote information from a source.

    One Author: Use the authors last name and the year of publication.

    • Example:
      (Smith, 2020)
      or
      Smith (2020) explains that effective communication improves teamwork.

    Two Authors List both authors last names every time.

    • Example:
      (Johnson & Lee, 2021)
    • As Johnson and Lee (2021) explained..

    Three or More Authors: List the first authors last name followed by et al.

    • Example:
      (Garcia et al., 2019)

    Proper Use of In-Text Citations Within Paragraphs

    Each paragraph should primarily reflect your own understanding, supported by evidence from scholarly sources. Citations should appear immediately after the sentence that uses information from a source.

    A common mistake students make is copying or closely paraphrasing multiple ideas from a source and placing one citation at the end of the paragraph, assuming it applies to the entire paragraph. In APA style, this is incorrect. A citation only applies to the specific sentence or idea it directly follows.

    Example of a Properly Written APA Paragraph

    Effective communication is a critical skill in both academic and professional settings. Research indicates that clear communication improves collaboration and reduces misunderstandings among team members (Smith, 2020). In academic environments, students who actively practice written communication often demonstrate stronger critical thinking skills because they must analyze and restate information in their own words (Johnson & Lee, 2021). Additionally, using proper citation practices helps students distinguish between their own ideas and those of others, which supports academic integrity and ethical scholarship (Garcia et al., 2019). By paraphrasing information and citing sources within each sentence, writers demonstrate understanding rather than simply repeating information from a source.

    Example of an Incorrect APA Paragraph

    Effective communication is important in academic and professional settings. Communication helps people work together more effectively and reduces misunderstandings. Students who practice writing improve their critical thinking skills and learn how to use sources ethically. Proper citation is important for avoiding plagiarism and maintaining academic integrity (Smith, 2020).

    Why This Paragraph Is Incorrect

    • The paragraph appears to rely on multiple ideas from a source, but only one citation is provided
    • The citation does not clearly identify which sentences are supported by the source
    • It assumes one citation can cover the entire paragraph, which is not correct in APA style
    • The paragraph does not clearly distinguish between the students ideas and the authors ideas

    APA Formatting Guidelines

    Font, Spacing, and Layout

    • Font: 12-point Times New Roman
    • Spacing: Double-spaced throughout the entire document
    • Margins: 1 inch on all sides
    • Alignment: Left-aligned
    • Paragraphs: First line indented 0.5 inches

    Formatting the References Page

    The References page provides full publication information for every source cited in the paper.

    • Start on a new page
    • Title the page References (centered at the top)
    • Double-spaced
    • Use a hanging indent for each entry
    • List sources in alphabetical order by the first authors last name

    Final Reminder

    APA format is not simply a set of rules to followit is a tool that helps you communicate clearly, ethically, and professionally. By paraphrasing in your own words, citing sources correctly within your paragraphs, and formatting your work properly, you demonstrate academic responsibility and critical thinking.

    When in doubt, consult your APA guide or ask for clarification.

    Again the course in NUR507 children and family but this clinical is pediatrics for children and adolescents meaning that the soap not MUST be on a child.

    A SOAP note is a method of documentation employed by healthcare providers to record and communicate patient information in a clear, structured, and in an organized manner. This assignment will provide students with the necessary tools to document patient care effectively, enhance their clinical skills, and prepare them for their roles as competent healthcare providers.

    Instructions:

    SOAP is an acronym that stands for Subjective, Objective, Assessment, and Plan. The episodic SOAP note is to be written using the attached template below.

    For all the SOAP note assignments, you will write a SOAP note about one of your patients and use the following acronym:

    S = Subjective data: Patients Chief Complaint (CC).
    O = Objective data: Including client behavior, physical assessment, vital signs, and meds.
    A = Assessment: Diagnosis of the patient’s condition. Include differential diagnosis.
    P = Plan: Treatment, diagnostic testing, and follow up

    Submission Instructions:

    • Your SOAP note should be clear and concise and students will lose points for improper grammar, punctuation, and misspellings.

    Your first SOAP note assignment is due this week. I cannot stress the importance of reading and following the rubric accordingly. Please note that these assignments carry a heavy weight on your grade and there will be NO RESUBMISSIONS after due date. If you have any questions, please reach out ahead of time.

    Keep in mind that the clinical course is a required component of NUR507. If you fail 1 component, both will be considered failures altogether.

    Tips for SOAP note assignments:

    • Do NOT choose a patient who came for a follow-up/wellness visit/annual check-up visit.You must select 3 patients out of the 125 that you must see throughout the term. Select a patient that has medical conditions that allow you to properly develop your note. If you cannot develop a required section of the SOAP note because you chose a patient who came for a follow-up/wellness/annual check-up visit, then you will not get points for that section. Do not use a template that you already used and submitted on previous courses. Start adding the information on a new template.
    • Read the rubric and address it accordingly. Do not have an empty section in the template.
    • Do NOT document normal or WNL as an assessment finding
    • Review the difference between ROS & physical examination (remember that in your ROS you only document subjective data and in the physical exam you only document objective data)
    • Turn-it-in score must be 20% or less (more than 20% will result in a “0”). AI report will be run for all SOAP notes.
    • Do NOT alter the template – do not remove or add sections
    • Look at the LAB(S) component of the rubric and find where to document that on the template (at the end of the physical examination)
