  • Decision making for the Advanced Practice Nurse

    For this assignment:

Consider that you are the provider on duty at a local urgent care clinic. As you read each of the patient scenarios, think about what the non-emergent diagnosis might be for this patient. Then consider this patients presentation and situation in light of emergency, life threatening, or life altering diagnoses. Answer the following questions for each patient:

#1 Identify which of the provided diagnoses should be assigned to each patient.

#2 Identify what signs and symptoms give you a clue that the disease process might be more serious than it presents.

#3 Is it appropriate to continue to treat this patient or do they require a higher level of care? If they require a higher level of care, please explain your rationale.

Meet your patients for the day:

Im having trouble breathing! That is certainly a phrase that will get the attention of a healthcare provider, but what does it mean? Is the patients nose stopped up? Are they having a panic attack and having only the sense of shortness of breath? Or are the lungs experiencing reduced oxygen exchange? Is the cause infection, impaired circulation, a mechanical injury, or something else? What body system is causing the issue? The only thing for certain is that we need to act quickly to figure it all out. As you know, time is of the essence.

Whitiker Carson, an 13-year-old male, was just brought to the clinic by his father. Whitiker has been sick for a few days with a fever ranging from 100.8 to 102.5. He has a snotty nose and is coughing frequently. The first few days his mom assumed he had a cold, but now Whitaker doesnt want to eat and is not even interested in his new gaming system. His cough sounds thick and he hasnt been able to rest because of the coughing. He says that when he coughs it makes his chest hurt, and he looks like a sick kiddo.

Approximately 30 minutes ago, Carlos Feliz, a 17-year-old high school basketball player, was jumping for a layup and had sudden intense pain in his left chest. He presented to the clinic with visible tachypnea and in a state of agitation. He is holding his left chest and keeps whispering that he cant breathe. Vitals: Height 66, Weight 152 lbs., B/P 108/68, HR 104, O2 on RA 94%, Resp 30, Temp 37.2

Rebecca Winsdale, a 36-year-old female, complains of shortness of breath and cough. She is training for a marathon next month but cant seem to control her breathing. She has used her rescue inhaler several times today already but doesnt think that it is doing much. She has heard a few wheezes, but the dry cough seems to be her chief complaint because it is throwing off her exercise regimen.

Marlin Corest, a 68-year-old male, was brought to the clinic by his daughter. She came to visit him earlier and thinks that something is very wrong. She states that his color doesnt look good, and he is not himself. She couldnt even get him to eat the cinnamon roll she brought for him. Mr. Corest has smoked cigarettes since he was 14 years old and was diagnosed with COPD last year, but this is an acute change. He is coughing up a lot more sputum, and the daughter says that Mr. Corests finger oxygen machine showed 84 when she got to his house.

Carla Shinto, the 51-year-old last patient of the day, arrived at the clinic with a complaint of shortness of breath, cough, and extreme fatigue for several days. However, she stated that something changed in the night last night. She is standing up, leaning against the exam table with outstretched arms when you enter the room. When asked about whether the cough was productive, the patient stated that she has been coughing up foamy pink stuff, but since its not green she doesnt think its an infection. She additionally stated that her husband has been complaining that her breathing is noisy, and she has been sleeping propped up on too many pillows. She is not resting well and gets winded just getting up to go to the bathroom. Is this a respiratory problem or is there more to know? Vitals: B/P 146/90, HR 94, O2 sat on RA 91%, Temp 37.4, Resp 28

Please note that there are more answer options provided than the number of scenarios presented. It is essential to carefully read each scenario in its entirety before selecting your answer. Use critical thinking and refer to your textbooks and clinical resources to guide your decisions. Focus on identifying key details in the scenarios that align with the best clinical response. This approach will help ensure accuracy and deepen your understanding of the concepts being assessed.

The purpose of this assignment is to raise awareness of your specific APRN role and specialty through an infographic. Imagine you are describing your APRN role and specialty to a high-schooler who might be interested in the same career. What would be important for them to know? Your infographic should be engaging, visually appealing, and informative. The infographic should convey your information clearly and concisely. Consider including some of the following facts to include: Basic demographics (How long has your role been around? How many NPs of your specialty are there? Where are the majority of the NPs in your specialty located? etc…) Educational/licensure/certification requirements (How long does it take to become one? How do we know you’re qualified?) Common work settings (Where can we find you?) Average salary (How much money do you make?) Services provided/types of patients & visits (Who do you see?) Commonly treated conditions (What do you do?) Associated organizations– certifying body, professional specialty org, etc (How do you organize? How do you stay up to date?) Specialty specific advocacy efforts/causes (What’s important to you?) Anything else you think is important Before you begin… Take some time to plan out your infographic before you get started. Make sure to review the rubric. As we move through the modules and you come across information you would like to include in your infographic, write it down. Sketch out a draft infographic before you get started. Creating your infographic… Recognize that infographics are meant to be read quickly. Include short bites of information, not entire paragraphs of text. Consider where you may be able to replace text with an image or graph. Your infographic must include: A title At least one graph or data visualization At least 4 images 8 – 10 facts
  • Accountability and Compliance in Advanced Nursing Practice R…

Advanced-Level Nurses: Accountability as Leaders of Change For this assignment, you will complete the provided Unit 8 Template. Differentiate the professional identity of advanced nursing practice in various roles, including accountability to the individual, society, and the profession and compliance with relevant laws, policies, regulations, and evidence-based practice. A few sections must include some content on identified topics. Address each section. You will research topics related to each content. For example, the APRN has a state RN license, a state APRN license, and a national certification. Policies may be related to a facility or may be governmental agencies. The nurse educator will have to comply with and understand FERPA laws, university policy, and standards for accreditation. Executive leaders must understand HIPAA implementation, wage and hour rules, and fair employment practices. Provide brief examples for each section. This is a discovery assignment. This activity will broaden your understanding of compliance and accountability in your role and other advanced-level nurse roles. Advanced leaders have an accountability to be compliant with evidence-based practice standards. While guidelines and standards may be slightly different and even altered based on a patient’s need, there are expectations to follow within reason. You will research evidence-based practice expectations and the importance of compliance per role. Provide three examples of concept implementation per block of the template. Bullet the information and cite within the blocks of the template. You will likely have at least one source per block. You can use the same source as applicable. The table will expand as you enter information if needed. You may single space as long as there is a clear delineation of examples. Content should be neat and rephrased, not pasted from sources. You do not need a title page if you complete the information at the top of the template. Assignment Requirements: Before finalizing your work, you should: Be sure to read the assignment description carefully (as displayed above). Utilize spelling and grammar checks to minimize errors. Your writing assignment should: Follow the conventions of Standard English (correct grammar, punctuation, etc.). Be well-ordered, logical, and unified, as well as original and insightful. Display superior content, organization, style, and mechanics. Use APA 7th edition format.

  • THE PSYCHIATRIC EVALUATION AND EVIDENCE-BASED RATING SCALES

Assessment tools have two primary purposes: 1) to measure illness and diagnose clients, and 2) to measure a clients response to treatment. Often, you will find that multiple assessment tools are designed to measure the same condition or response. Not all tools, however, are appropriate for use in all clinical situations. You must consider the strengths and weaknesses of each tool to select the appropriate assessment tool for your client. For this Discussion, as you examine the assessment tool assigned to you by the Course Instructor, consider its use in psychotherapy.

    TO PREPARE:

    • Review this week’s Learning Resources and reflect on the insights they provide regarding psychiatric assessment and diagnosis.
    • Consider the elements of the psychiatric interview, history, and examination.
    • Consider the assessment tool assigned to you by the Course Instructor.

    Assessment Tool assign

    Brief Psychiatric Rating Scale (BPRS)

Post a brief explanation of three important components of the psychiatric interview and why you consider these elements important. Explain the psychometric properties of the rating scale you were assigned. Explain when it is appropriate to use this rating scale with clients during the psychiatric interview and how the scale is helpful to a nurse practitioners psychiatric assessment. Support your approach with evidence-based literature.

Introduction – Family Disaster Plan Scenario

    1. You must include research on the dangers and explain the recommended safety measures in a tornado emergency.

How would you prepare for the following situation?

(Scenario) At this moment, you are sitting at home working on your WCU class. Suddenly, the National Weather Bureau sends an alert across your cell phonea tornado is headed your way. You have 15 minutes before touchdown in your neighborhood.

    1. What is your plan? This is a ‘shelter in place’ scenario, you cannot outrun the tornado. Identify a safe place in your home (residence) to take shelter.
    2. Provide realistic examples and explain how you would apply the safety and survival measures you learned in your research of a tornado emergency to your specific living arrangements.

Example: I will turn off my utilities before I shelter in place to mitigate damage to my residence.

Example: I will take by go-bags with me to my shelter in place.

How prepared are you in the event of a disaster?

    1. Describe your level of disaster preparedness using specific examples and references to:
    2. Potential disasters in your area.
    3. Your 72 hour “go-bag” assignment.
    4. Family Disaster Plan Checklist.

Example: I am more prepared for a water-related disaster than a fire-related disaster even though I live in a highly secluded, forested area. I have a boat as transportation in the event of flooding, but I do not have rain barrels or fire barrier supplies on hand.

Example: “There were many missing items on my preparedness checklist. I realized that I do not own a flashlight. If I had to use my phone as a light it would drain the battery very quickly.

    Reflection/Conclusion

    1. Reflect on how prepared you were before this class and compare it with how prepared you are now.
    • Have you acquired any new emergency items?
    • Do you plan to take any additional training or certification courses?
    • Have you shared your knowledge with friends and family?

    Content Requirements

Research TOPIC: Rising incidence of gestational diabetes mellitus (GDM) in pregnant women. What is the impact of a family nurse practitioner-led evidence-based educational program for women at risk of developing gestational diabetes mellitus in a primary care clinic?Follow this structure in your presentation:

    1. Research Problem (Slide 12)

    • Identify a current issue in advanced nursing practice.
    • Explain why it is a problem (in 3 sentences or fewer).
    • State your research question clearly.

    2. Research Design (Slide 3)

    • Indicate whether a qualitative or quantitative study best answers your question.
    • Justify your choice.

    3. Middle-Range Theory (Slide 4)

    • Select and briefly describe a middle-range nursing theory.
    • Explain its relevance to your chosen research problem.

    4. Literature Review and Study Critique (Slides 512)

Utilizing your Literature review from earlier in the course, summarize and critique the relevant studies based on its design.

    If Qualitative:

    • Study purpose and design
    • Ethical considerations
    • Sampling and recruitment
    • Data collection and analysis methods
    • Rigor (4 criteria: credibility, dependability, transferability, confirmability)
    • Key findings and limitations
    • Trustworthiness and nursing implications

    If Quantitative:

    • Study purpose and design
    • Blinding and randomization
    • Ethical considerations
    • Sampling method and sample size
    • Data collection instruments and procedures
    • Validity and reliability
    • Statistical analysis and significance
