the scenario: This report presents a clinical scenario encountered in the intensive care unit (ICU) involving a patient diagnosed with brain death following emergency admission after being found unresponsive at home by her family. The patient underwent emergency intubation to protect the airway because her level of consciousness is low (low GCS) and diagnostic investigations, including a CT scan of the brain, which revealed severe neurological injury and dilated pupils. The final medical diagnosis confirmed brain death, and a Do-Not-Attempt-Resuscitation (DNAR) order was initiated.
The purpose of this report is to analyze the ethical and clinical issues in this case, explore the decision-making process in end-of-life care, and discuss the conflict between healthcare professionals and the patients family who refused to accept the diagnosis and DNAR order.
