Activities (S-CATsc Evaluation & Improvements)

Form a group of 3-4 people

Complete S-CATsc collectively (e.g., majority votes)

Identify leading indicators that need improvements

Select most appropriate resources from SC-SMIS repository to

achieve the improvements

Establish action plans (files are in the module)

Name all participating teammates in the notes (Teammate A, Teammate B, Teammate C)

Submit the action plan pdf file.

Use template attached to fill out the action plan. Follow directions in the attached slides. See sample attached action plan for reference.
