    During my time working as pct on medsurg unit, Ive been taking care of many patients in comfort measures only, including a patient with end stage of cancer, who frequently experienced anxiety, discomfort because of pain, shortness of breath, and fatigue. I applied Kolcaba’s Theory of Comfort, which defines holistic comfort for nursing, which she represents as three types: relief, ease, and transcendence (Kolcaba and Wilson, 2002); and four contexts: physical, psychospiritual, sociocultural, environmental (Kolcaba et al, 2006). To promote relief and physical comfort, I made sure she always received her pain medication on time, ensured her O2 was set up and available whenever she needed it for her shortness of breath, and provided heating pads and warm blankets by her side. During daily baths, I allowed her to take multiple breaks whenever she felt fatigue and made sure it was done efficiently so she wouldn’t get a chill. With ease, psychospiritual and sociocultural, I was spending time talking to her when possible, to reduce her anxiety, and make sure she had her emotional supporting stuff from her family with her or in her view. Lastly, I make sure to give her a calm environment, a quiet space with dim light, and a warm blanket to rest. Overall, these interventions I provided followed Kolcabas theory and supported patient-centered comfort care.

    Nursing Theory Application

    I do not have any healthcare experience and I have not done any clinicals in school yet. For this assignment I did think about an experience involving caring for my grandmother. My grandmother is 93 years old and is currently living with my family and I. She has dementia and struggles with remembering and understanding information. My grandmother is unaware of her age, does not recognize faces of her family members, and her memory is limited to a short period of time. She often will wake up from bed in a panic not knowing where she is and how long she has been there. When she is greeted by a family member she will often tell them they are the first person she has seen in weeks. She lives in a constant state of confusion and does not have any memory to ground herself in any secure feeling. The theory I will be selecting to discuss is the Comfort Theory from Katherine Kolbaca. This theory discusses how in order for someone to have increased health seeking outcomes they need to be comfortable first (Minter, 2026). Concepts of the theory could be applied to this situation by providing as much ease and relief as possible. Meeting her where she is and letting her know she is being cared for and wanted, would provide a sense of security despite confusion. Giving her that sense of safety and assurance is enough to get her back to bed and be comfortable with her state.

Annotation Example In this article, Mezirow (2003) makes a distinction between “instrumental” and “communicative” learning. “Instrumental learning” refers to those processes which measure and gauge learning, such as tests, grades, comments, quizzes, attendance records and the like. “Communicative learning,” on the other hand, refers to understanding created over time between individuals in what Mezirow calls “critical-dialectical-discourse,” (p. 59) which is a fancy way of saying, important conversation between 2 or more speakers. Another key idea Mezirow discusses is “transformative learning,” (p. 61) which changes the mind, the heart, the values and beliefs of people so that they may act better in the world. Mezirow argues that “hungry, desperate, homeless, sick, destitute, and intimidated people obviously cannot participate fully and freely in discourse” (p. 59). On the one hand, he is right: there are some people who cannot fully engage because their crisis is so long and deep, they are prevented. But, I don’t think Mezirow should make the blanket assumption that everyone in unfortunate circumstances is incapable of entering the discourse meaningfully. One thing is certain: if we gave as much attention to the non-instrumental forms of intelligence–like goodness, compassion, forgiveness, wonder, self-motivation, creativity, humor, love, and other non-measured forms of intelligence in our school curriculums, we’d see better people, actors in the world, and interested investigators than we currently have graduating high school.
