    Instructions:

    Read the attached article, ‘Investigating Adverse Events in Long-Term Care Facilities’ (Module 2 reading). Then review the following mini-case study scenario and answer the critical thinking questions.

    Mini-Case Study Scenario:

    You are the Patient Safety and Quality Coordinator for a long-term care facility. Your facility has recently experienced an increase in adverse events related to falls and medication errors, resulting in resident harm and survey deficiencies. Leadership is prioritizing the improvement of patient safety culture and practices across the facility. You have been asked to draft a brief action plan describing how your facility can investigate and reduce adverse events to improve resident outcomes.

    Critical Thinking and Solution Identification:

    1. Using 5-Why root cause analysis, identify at least two root causes of the adverse events occurring in your facility (one related to falls, one related to medication errors) and explain why these errors or gaps occur.
    2. Describe two specific programmatic or operational changes your facility could implement to reduce adverse events in alignment with patient safety improvement frameworks.
    3. Explain how data/information technology could be used to monitor and report progress on your plan to reduce adverse events.
    4. Discuss how your plan would help the facility meet applicable accreditation or licensing standards related to patient safety and quality in long-term care.
    5. Cite at least one relevant point from the assigned article to justify your proposed approach. Cite the point in APA format within the text of your presentation and as part of a reference page/slide at the end of the presentation.
    6. Find a supporting reference to your suggested operational change. The article must be peer-reviewed and from a source ranked on the JANE journal guide (https://jane.biosemantics.org/faq.php). Cite your supporting point in APA format.

    Each student will also write a minimum 1800 word paper that thoroughly explains his or her theological position regarding the particular contemporary cultural issue addressed.
