Case study

Case Study:

A 32-yo woman has just given birth to a 36-week newborn after prolonged labor due to shoulder dystocia. The baby was born apneic, with minimal muscle tone. HR was 80. The newborn was attended to immediately by the neonatal resuscitation team, who provided PPV for approx. 30seconds. The newborn began breathing spontaneously and was then transferred to the NICU on supplemental O2. Three hours later, despite O2, the baby was still cyanotic. The physician ordered pulse oximetry of the right hand and left foot, which revealed saturations of 96% and 75% respectively. Analyze this case study in an essay format of at least 500 words, making sure to address the following questions:

  1. What are some of the issues that may be presenting in this case?
  2. At what stage are the lungs fully developed to maintain homeostasis in extrauterine life?
  3. What two anatomical shunts normally present in the fetus can result in persistent cyanosis?
  4. Why would the newborn in this case be at risk for refractory hypoxia?
  5. Why are the oximetry readings necessary, and why are the probes placed at the sites specified by the MD?
