The final project for this course is an analysis of a hospital, state, or federally funded healthcare organization with recommendations that will evaluate the cultural competence of an organization and the ways it provides services from a population health approach framework. It is important for healthcare organizations to use strategies for improving services to a diverse population in order to provide quality care to all patients.
A population health approach unifies, integrates, and balances prevention and promotion, health protection, diagnosis, treatment, and care. It acknowledges and reflects evidence that factors outside of the healthcare system can significantly affect health. It considers the entire range of individual and collective factors and conditionsand their interactionsthat have been shown to be correlated with health status. Commonly referred to as the determinants of health, these factors currently include income and social status, social support networks, education, employment/working conditions, social environments, physical environments, personal health practices and coping skills, healthy child development, biology and genetic endowment, health services, gender, and culture.
A population health approach focuses on improving the health status of a population, rather than of individuals. Such a focus necessitates reducing disparities, or inequalities in health status among population groups. The outcomes or benefits of a population health approach, therefore, extend beyond improved population health outcomes to include an integrated health system and increased social cohesion and citizen engagement.
The project is divided into three milestones which will be submitted at various points throughout the course to scaffold learning and ensure quality final submissions. These milestones will be submitted in Modules Three, Four, and Six. The final project will be submitted in Module Seven.
In this assignment, you will show your ability to complete the following course outcomes:
Select a hospital, state, or federally funded healthcare organization. Based on what you learned and the course material, you will evaluate the cultural competence of the healthcare organization.
Specifically the following critical elements must be addressed:
Milestone One: Patient Engagement Activities and Communication Strategies
In Module Three, you will submit Section I of the analysis, in which you will address the patient engagement activities and communication strategies of the health organization. Specifically, you will critique the organizations translation services, educational materials, and staff training options. You should consider things such as the variety of language options, how well the organization meets the language requirements of its service demographic, and how well staff training addresses key cultural competence issues. This milestone should be two to three double-spaced pages. This milestone is graded with the Milestone One Rubric.
Milestone Two: Health Promotion and Disease Prevention and Management Strategies
In Module Four, you will submit Section II of the analysis, in which you will address the organizations health promotion and disease prevention and management strategies. Specifically, you will critique the organizations healthy living programs, disease prevention and management programs, and non-programmatic services. You will assess the cultural competence of these services using the population health approach. Be sure to consider issues such as how well the organizations health promotion and management strategies address a wide range of cultural beliefs and whether the organizations programs and non-programmatic services help to reduce inequalities among members of the population. This milestone should be two to three double-spaced pages. This milestone is graded with the Milestone Two Rubric.
Milestone Three: Financial Incentives and Quality Improvement Processes
In Module Six, you will submit Section III of the analysis, in which you will address the organizations financial incentives and quality improvement processes. Specifically, you will critique the organizations population-health-oriented policies, the organizations approach to care and the extent to which it is patient-centered, and any strategies the organization uses in its population health approach. This milestone should be two to three double-spaced pages. This milestone is graded with the Milestone Three Rubric.
Final Submission: Case Study Analysis and Proposal
In Module Seven, you will submit your analysis and proposal using a population health approach framework. It should be a complete, polished artifact containing all the critical elements of the final product. It should also reflect the instructor feedback for each milestone. This submission is graded with the Final Project Rubric.
Written components of the project must follow these formatting guidelines: double spacing, 12-point Times New Roman font, one-inch margins, and APA-style citations. The analysis and proposal should be 8 to 10 pages in length, not including the cover page and resources.

This project asks you to explain the cultural
meaning of an object. You should choose and object that has some importance to the culture of Rereading Americasome
object that relates to the homelife described in the short essays you read from section 1,
Harmony at Home or the ones you plan to read from Learning Power in section2. It can also be an object that relates to your major, in which case you will need to do research or have personal experience with that object in that field.
This project is about meaning.
It is not about the history of the object, the factual elements are only to support your
ethos as you make your claim about what the object means. Your paper should focus on how the artifact from the field represents the meaning of your field. Think of the farmer, how that Ram commercial is predicated upon the meaning of farmer and how it ties Ram
trucks to that meaning. But you must
be more honest than the commercial and explain explicitly how the artifact that is your focus represents the meaning of the field.
To write this paper you will need to think about and research the object so that you can have
facts to support the ethos of your claims about what it means.
DO NOT fall into the trap of just reporting those facts though.
Your research should allow you to say things that most who at first think about your profession and artifact wouldnt think of. Your paper uses these facts to make yourself sound like an expert about the object.
LEARNING GOALS:
1) to invite you to inquire into cultural values and/or practices of
literacy (what does it mean to know about a thing?) by choosing an artifact as the focus of
your inquiry; and 2) to give you practice in processes of inquiry: forming and asking
questions, revising questions, and seeking answers.
RHETORICAL PURPOSE:
of the project is to demonstrate the significance of an everyday
item for understanding a larger pattern of cultural values or practices. Your reader should
come away from your essay having learned a new way to look at taken-for-granted artifact
through the cultural values/practices you discover in your analysis of the artifact youve chosen.
THE FIRST STEP:
Learn about the object. To learn about its material, literal, concrete properties. It is important here to dig until you go past the surface, the obvious. You should reach
a point where you discover facts about the object in question that you did not know going in.
These facts become your Ethos as you then describe the previous usage and cultural (figurative)
meaning of the object and the changing usage and how that new usage changes its meaning.
THE GOAL:
Take a common object and research it until its literal and figurative strangeness is revealed.
