Purpose Statement:

This assessment task will enable you to gain skills in

a) applying validated wound assessment tools to wound assessment tasks.

b) talking to people, to gain an understanding of their needs and factors that may affect their capacity to heal.

c) refining your professional communication skills by communicating your assessment findings to other members of the multidisciplinary team.


Learning Outcomes

This task is assessing the following LO (taken from the handbook):

LO1 – Apply a holistic approach to the systematic assessment of patients with wounds, incorporating physical, psychosocial, cultural and environmental factors.
LO2 – Identify and apply existing assessment strategies to inform a wound diagnosis and communicate this to the patient, their carers and the relevant multidisciplinary team(s).
LO3 – Formulate person-centred goals of care, in consultation with patients and their carers based on a holistic assessment of a patient with a wound.
LO4 – Distinguish how cross-cultural values and ethical requirements impact on the assessment of patient with wounds.
LO5 – Recognise and apply professional communication strategies when advocating for patients who require multidisciplinary team management.

3. Details of Task

There are three distinctive sections to this assessment task. It is fundamental that each section is considered in the whole portfolio.

  • You do not have to label your final word document in the same way, this is optional in the final formatted submission.
  • If you are not currently working clinically and need assistance with a case, please email the Unit Coordinator, who can assist you to provide a case study.


4. Section 1

Find one person with a wound, from within your workplace and invite them to participate in your assessment activity. This person must be different from the person you assessed in Part A of this assessment task. In completing this activity, you will need to ensure that the person (or Medical Power of Attorney) consent to an interview and the taking of images for an assessment of their wound. (If the person has multiple wounds, just focus on one for the purposes of this assessment).

There will be two ways in which you will conduct Section 1:

1. An individual interview: carefully consider the type of questions you will ask the person and provide a rationale for why the question was asked what information will you hope to achieve by asking this question series? You do not need to include the responses in this section. The relevant responses should be incorporated into your full assessment of the person.

2. A collection of images of the individual persons wound, inclusive of a measure tool. Ensure to include an “orientation” image that shows the location of the wound on the limb, and a close-up image of the wound itself. Please respect the persons dignity and hide any distinguishing features or sensitive areas.

Once you have conducted the interview and collected images proceed to Section 2.

5. Section 2

For the person:

1. Write a summary of the persons:

  • wound history
  • social history
  • medical history (including medications types, dosage and frequency)
  • relevant information gained during your interview
  • interactions with any other health professionals
  • the persons short and long-term wound care goals

2. Apply MEASURE assessment to the person’s wound and critically appraise your assessment by providing a wound aetiology with justification. Use evidence from the literature to support your statements.

3. Identify two (2) factors from the persons medical, social and/or wound history and critically analyse how those factors impact on the individual’s capacity to heal, and if any of these factors can be modified. Use evidence from the literature to support your statements. These two factors must be different to the wound aetiology.

4. Provide a short (2 minute) audio recording in which you explain, in layperson language, the aetiology of the wound and two factors that may affect this persons ability to heal, as if you were speaking directly to them. Record this using the Sound Recorder app on a PC, or Voice Memos app on a Mac and then insert the audio file into your Word document.

5. Formalise two (2) short-term and two (2) long-term person-centred goals which are specific, measurable, achievable, relevant and timely for the person and environment.

6. Section 3

Based on your interview and assessment, write a request in the form of a letter to another health professional (e.g., general practitioner, community nursing service, vascular surgeon, podiatrist etc).

This request can be for review, for ongoing care, for assistance with diagnosis, whatever is required for the person. In this letter you must include all aspects of the individual which are necessary to address your needs in the request.

The letter should be comprehensive yet succinct (no more than 800 words).

7. Instructions for Submission

Your written submission must be submitted as a Word document and contain the aspects listed below. Failure to submit all of the following will result in either marks being deducted or a failure on this task.

  • Your full assessment of the person, inclusive of the images you have taken.
  • All interview questions and their rationale. You do not need to include the responses; these should be incorporated into your full assessment of the person.
  • Copies of the signed consent form, included as an Appendix. If signed consent forms are not included as Appendix, your assessment will not be marked.
  • Any other evidence that may support your assessment of the patients (i.e., de-identified pathology results, medical imaging, vascular studies)
  • You are not required to include an introduction or conclusion in the submission.

Ensure that your final submission is appropriately cited, and a reference list is included in accordance with APA 7th edition (links in Resources folder).

8. Consent Form

This is the CONSENTFORM which you will need to have your patient complete and include in your submission. If your patient is unable to consent, their Medical Power of Attorney may sign on their behalf using the same form.

9. Structure and style

This task should be completed using a Word document and follow the formatting suggested in the Master of Wound Care Course Guide.

The use of sub-headings is recommended.

10. Marking Criteria

The marking criteria will assess your ability to:

  • Critically appraise and apply clinical judgement wound assessment practices.
  • Demonstrate a holistic approach to the assessment of the person.
  • Correctly apply the MEASURE tool, and
  • Your ability to professionally communicate with a person with a wound.

You will receive individual, written and verbal feedback via the Rubric on Moodle.

11. Rubric

The rubric for the portfolio is attached separately.

Please attach the consent inside the word document with the name of the patient from the case.
Please provide a referral inside the word document.
