Course Objectives:

The course objectives should be listed on the first page of the Clinical Log document.

Each summary log entry should refer to these course objectives:

Upon completion of this course, the student will be able to:

1. Integrate the role components of the nurse as provider of care, leader and manager, and member of

profession.

2. Promote the health and well-being for persons in dynamic environments through professional nursing

practice.

3. Describe the impact of environment on health and well-being of persons and nurses.

4. Communicate the nursing perspective within the process of interprofessional collaborative practice.

5. Apply principles of evidence-based practice to deliver quality care in a diverse, global society.

B. Personal Objectives

Each student will write their own personal objectives prior to beginning the practicum clinical experience and

refer to them with each summary log entry. The students personal objectives should be listed on the first

page of the Clinical Log document.

Personal objectives should take in account the students individual strengths and limitations, be consistent

with the course objectives, and reflect what the student plan to achieve during practicum. They are not a list of

clinical skills.

SAMPLE personal objectives:

1. Develop interpersonal skills with health team members.

2. Provide comprehensive discharge teaching instructions to patients and families.

3. Perform complete, clear, and concise documentation.

C. Clinical Log – Entries 1, 2, and 3

Log Entry When to Complete Due

Log Entry 1 After 36 hours with preceptor Within 72 hours of completing 36 hours

Log Entry 2 After 72 hours with preceptor Within 72 hours of completing 72 hours

Log Entry 3 After 107 hours with preceptor Within 72 hours of completing 107 hours

Include the following items in each Clinical Log entry:

3. EBP Article Application

1. Self-evaluation

Each Clinical Log entry should include a summary of the nursing care you provided during your clinical

hours and an evaluation of the care you delivered. You do not need to list every task or skill performed.

Instead, highlight the nursing actions that best demonstrate your progress toward the course objectives

and your personal learning goals.

In each log entry, you must clearly identify which course and personal objectives you addressed. If

you did not have the opportunity to meet a particular objective during that period, simply state that.

  • Example: I did not have an opportunity to meet Personal Objective #2 during my first 36 hours
  • of practicum, as we did not discharge any patients.

    Your self-evaluation should go beyond listing tasks. Reflect on your clinical decision-making and explain

    the thinking process behind your actions. Discuss how you applied classroom knowledge to real patient

    scenarios, solved problems or adapted to unexpected situations, and how you met or worked toward

    specific objectives through those experiences Use this reflection to demonstrate critical thinking and

    personal growth throughout the practicum.

    3. EBP Article Application

    For each of the 3 clinical log entries, locate an evidence-based practice (EBP) nursing or healthcare

    publication (single study or higher level of evidence) that relates to your clinical experience. In a few

    paragraphs:

  • Briefly summarize the article and its findings
  • Discuss what you learned from the evidence in your summary log entry and how you will apply
  • the evidence to your clinical practice

  • Submit a pdf copy of the EBP article with your Clinical Log submission on Canvas
  • Cite information appropriately using APA 7th ed. format.
