Your patient is being discharged from the hospital after being admitted for a stroke that has left the patient with right-sided weakness. The patient can feed and dress himself. He does require help with bathing and cooking. He requires a cane for ambulation. He will need more PT (at least three times a week). He refuses to go to a nursing home or assisted living facility for rehabilitation unless there are no other options. He currently lives in a single-level home. Your social worker is unavailable, so you need to assess your local area and include services available to the patient that may help him stay in his home. (Think of all areas…meals, transport, physical help, etc.) What services are available to the patient in your area? What services are lacking?
