He is only able to speak in short sentences and is slightly obtunded. His skin is slightly cool and clammy to touch. Urine output is < 20mL/hour. His oxygen saturation in the ER is 84% while on room air and heart rate is 143. His blood pressure is 138/99. You notice rapid breathing at a frequency of 37 bpm. You auscultate diffuse crackles throughout all dependent regions of the lungs. The patient is lying back in a recumbent position saying that he feels like he is suffocating. Arterial blood gas analysis results are pH-7.19, PaCO2-25, PaO2-52, HCO3– 22.
