Hearth disease prevention

Topic: Improve cardiovascular health in adults. Target Audience: adults 40 years old and older Topic: Improve Cardiovascular Health in Adults (Age 40+)

Framework: MAP-IT (Mobilize, Assess, Plan, Implement, Track)

Step 1: Understand the Topic

Focus on cardiovascular health in adults aged 40 and older, including prevention and management of conditions such as hypertension, high cholesterol, diabetes, and overall heart disease risk.

Step 2: Use the MAP-IT Framework (Required)

Your entire project must be organized using the MAP-IT framework provided: Mobilize (identify and engage stakeholders such as pharmacists, physicians, community organizations, patients, and families), Assess (identify major health issues, risk factors, and barriers such as lifestyle, access to care, and medication adherence), Plan (develop clear goals, measurable objectives, and realistic interventions), Implement (explain how and where your plan will be carried out and how patients will be engaged), and Track (describe how you will measure outcomes and make improvements).

Step 3: Complete ALL 3 Required Components

You must complete three components for this project. First, create a PowerPoint presentation with a minimum of 1520 slides that follows the MAP-IT structure; slides should be concise and not overly wordy, and you must include detailed speaker notes that are separate from the slides. Second, create a patient-friendly pamphlet that is simple, clear, and easy to read, designed specifically for adults age 40+, and includes key information such as risk factors, prevention strategies, lifestyle changes, and when to seek care. Third, create a poster that is visually appealing, easy to understand, and targeted to adults age 40+, highlighting the most important cardiovascular health messages.

Step 4: Important Requirements

All work must be tailored to adults age 40+, clearly reflect the MAP-IT framework, incorporate the role of the pharmacist, and use realistic, practical strategies. Be sure your content demonstrates a strong understanding of public health concepts, and include relevant data or references if required.
