  • Sickle Cell

Good day, The instructions to the assignment are attached in the photo. It is to all be done in an apa format . Here are some key points:

Have a detailed introduction and conclusion, page numbers, apa header, table of contents page and appendices where applicable.

The topic we selected was sickle cell disease (SCD) so the information within this assignment should reflect this and the different types Hemoglobin SS (sickle cell anemia), Hemoglobin SC, and Hemoglobin S-beta thalassemia.

For #2: The teaching plan for our school has also been attached. Four should be done in total. Each should reflect the different aspects of the disease that would be important. For example, one on pain management, one on identifying signs and symptoms and preventing crises etc.

For #3: Create a brochure that includes apa in text parentheticals, and a list of apa references to the back. Also have Bahamian resources for people who have sickle cell. You have freedom with the design as long as its related to sickle cell.

For #4: The standardized form should include the information that was covered. However, since the public will be completing it, it has to have simple words and terminology.

For the other parts 1 and 5 also include in text apa parentheticals and have an apa reference list for the entire assignment. Be as detailed as possible without it sounding like were pHd students. Meaning that the parts where its asking you to identify weaknesses, strengths, identifying five key components etc. write *a lot* of information. Also touch on the view from a nursing managers standpoint.

Unless something is specified you can have freedom with it. I can also make small adjustments. Thank you.

  • discussion 7

The patient is a 71-year-old widowed man who is seen regularly in the clinic for health maintenance and follow-up of his chronic insomnia and anxiety. He has regular prescriptions for triazolam (Halcion) and clonazepam (Klonopin) for these problems. Recently he has been reporting frequent episodes of losing his balance and falling, and eight weeks ago was hospitalized for a hip fracture sustained during one of these falls resulting in hip surgery. On this visit, he also complains of becoming increasingly confused.

    • What information would be most critical for you to collect in the first visit?
    • What is the primary goal for the treatment of this patient?
    • Identify potential obstacles for change. Which educational approach would the PMHNP provide to overcome these obstacles?
    • How would you teach the patient about the Beers list and Halcion?
    • Discuss a medication in detail that could be safely substituted to treat insomnia in geriatric patients.
    • Your initial post should be at least 500 words, formatted and cited in current APA style.
    • Provide support for your work from at least 2 academic sources less than 5 years old.

  • Psychoanalysis and Ego Psychology in Nursing Psychotherapy

Identify and contrast which theorist developed psychoanalysis and psychoanalytical therapy? Compare and contrast classical psychoanalysis with ego psychology theorists. How are their views similar? How are they different? What are the expectations of clients who participate in classical or traditional psychoanalysis and what is the role of the therapist in this therapeutic approach (and why)? Describe transference and countertransference, and how the nurse psychotherapist can use these in clinical practice. What should you do if you experience this when counseling a patient? Purpose: The purpose of required threaded discussions is an interactive dialogue among instructors and students to assist the student in organizing, integrating, applying, and critically appraising ones knowledge regarding the nursing profession and selected area of practice. Scholarly information obtained from current sources as well as professional communication is required. The articles should have been published within the past 5 years and be peer reviewed. In some cases, you will need to pull in content from the American Psychiatric Associations Diagnostic and Statistical Manual of Mental Disorders (DSM-5). Application of information to advance practice nursing experiences promotes the analysis and use of principles, knowledge and information learned and related to real-life clinical situations. Interactive dialogue among instructors and peers fosters the development of a learning community as ideas, perspectives and knowledge is shared.

Advocating to Educate on Community Resources to Support Social Determinants of Health

What are significant social needs in your community and how can you help? Through your professional role as a nurse, as well as personal interactions as a community member, you likely have insight into issues that impact certain populations. Chances are you also see from your daily interaction with patients how lack of information or information that is not easily accessible or understandable prevents patients from making changes or getting resources and support that would improve their lives.

Using your knowledge of the social determinants of health and the needs of particular groups, you can be a significant advocate for a healthier community. Think of yourself as a bridge that connects those who need resources with those who can provide them. Every organization can use another voice to spread the word about what they do and offer. Vulnerable populations will benefit from an advocate who is aware of potential barriers, such as language or literacy levels, in recommending community resources.

For this Competency Assessment, you will identify a particular population in your community. You will research this population to understand their specific characteristics and needs and match them to organizations and services in your community. In addition, you will identify missing services to be developed and make the case for strengthening existing services. You will organize your research using a PowerPoint presentation. (Note: You are not required to add audio narration.) Think of your presentation as a place to start in determining how best to reach and inform your focus population and in filling your role as a social change agent with the information you gather.

    To prepare:

    • Access the PowerPoint template document for your presentation. Reflect on how to create a presentation that could reach and inform stakeholders in your healthcare facility and your community.
    • Research to identify specific needs in your community based on the social determinants of health (SDoH): economic stability, education quality and access, healthcare access and quality, neighborhood and built environment, social and community context.
    • Identify a population (e.g., children, families, the elderly, those with a particular health condition, residents of a particular neighborhood) with specific SDoH needs that you will advocate for as a social change agent in your community. Research to identify key characteristics of this population that are important in identifying resources for them, such as age, language, cultural considerations, technology access, literacy levels, and so on.
    • Research resources in your community (e.g., organizations and services) you would recommend to the target population to address their specific SDoH needs and improve health and well-being. Aim for resources that will address each SDoH as it impacts your focus population or the SDoH(s) of greatest need. Identify all important information that the population will need to access and benefit from each resource to include the following: location, hours, languages spoken/multilingual materials available, special services, free services/materials, online links. Make a point to learn as much as you can about the resource(s) you highlight, including to determine ways that services could be improved to better meet your populations needs.
    • Based on your research, identify services that are missing in the community and needed by your population.
    • Consider strategies for reaching the focus population that align with characteristics of the population (e.g., technology access and forms, such as text, email, or print/poster options).
    • Based on your advocacy plan, formulate goals for improving health outcomes for this population.
    • Note: The PowerPoint template includes formatting notes and tips. Be sure to delete this information from your slides. Your presentation should feature only your original content and wording.

    To complete the Competency Assessment:

Using the PowerPoint template document provided, create a PowerPoint presentation of 810 slides, plus title and reference slides, to include the following:

Responding to Abnormal Physical Assessment Findings With Culturally Competent Care

One goal of this Competency is to acknowledge your experience and to help you examine, interpret, and learn from it in ways that advance your nursing skills. This Competency also puts priority on addressing aspects of nursing that are inevitable yet can be inherently challenging.

With those goals in mind, your Competency Assessment requires an example of an abnormal finding in the physical assessment of a patient. You are encouraged to draw from your own experience. However, you may also work with a focus case obtained from another professional nurse. Consider details, such as the following, that you will need to provide:

    • What was the purpose of the exam?
    • How was the abnormality found?
    • How was it determined to be abnormal?
    • Was the abnormal finding usual/unusual, otherwise characterized, based the patients age/place in the lifespan?
    • How was the finding shared with the patient?
    • What were next steps for diagnosis and/or treatment?

Explaining the role of cultural competence in nursing care is an equally important component of this Assessment. You will analyze the impact of cultural competence in conducting a physical assessment and responding to an abnormal finding. You will explain how a nurses demonstration of cultural competence would improve the patients experience and potential outcomes. For these requirements, you may incorporate details about your focus patient that you know. Or, you may approach this part of the Assessment by presenting best practices.

Your Performance Task is to draw this information on cultural competence and physical assessment together in a PowerPoint presentation to inform nurses in a practice setting on providing culturally competent care and the particular impact with physical assessments that have abnormal findings. (Note: You are not required to include audio narration.) Be sure to adhere to rules of confidentiality and omit identifying details about the patient, healthcare setting, or other healthcare providers.

Note: Be sure to delete the formatting notes and tips in the PowerPoint template. Your presentation should feature only your original content and wording.

    To complete the Competency Assessment:

Using the PowerPoint template document provided, create a PowerPoint presentation of 810 slides, plus title and reference slides, to include the following:

Click each of the items below for more information on this Assessment.

    • Explain what cultural competence means in professional nursing practice.
    • Describe examples of how culturally competent care benefits patients and patient outcomes.
    • Explain practical approaches to demonstrating cultural competence, including cultural humility, in nursing care.
    • Explain how knowledge of a patients culture, including norms and beliefs, is important in a physical assessment, and why.
    • Identify a patient who is the focus of your presentation with appropriate details: the patients age, cultural background, health background, and purpose of the physical assessment.
    • Explain what you found in the physical assessment of the patient, how you made the identification, and why the finding was abnormal. Please include specific content from the physical assessment – think of this as documenting your assessment findings in the practice setting. Provide support regarding how you know the assessment findings are abnormal.
    • Based on the patients age and place in the lifespan, explain how you would characterize this finding, and why. Provide specific changes that a person goes through as they age that can contribute to the assessment findings.
    • Explain how knowledge of the patients cultural norms and beliefs supports presenting the abnormal finding to the patient.
    • Explain your decision making on next steps for the patient.
    • If you were making a referral for this patient, explain what you would consider, and why.
    • Explain the impact and value of this experience on your nursing practice, with specific examples.

  • Benchmark Paper

One way informatics can be especially valuable is in capturing data to inspire improvements and quality change in practice. The Agency for Healthcare Research and Quality (AHRQ) collects data related to adverse events and safety concerns. If you are working within a practice setting to implement a new electronic health record (EHR) system, this is just one of the many considerations your team would need to plan for during the rollout process.

In a paper of 1,500-1,750 words, discuss the following:

    1. Consider an opportunity for tracking care improvement. What key information would be needed in the database? Example: Time lapse from medication order documented in the EHR to delivery of medication to the patient for the cardiology service.
    2. Describe the role informatics plays in the ability to capture these data. What type of project management strategies and methodologies can be implemented to support informatics initiatives to help improve quality within the clinical practice?
    3. Discuss which systems and staff members would need to be involved in the design and implementation process. What is the role of the advanced registered nurse in promoting evidence-based practice and leading quality and performance improvement initiatives within this setting?
    4. Outline strategies for implementing the new EHR proposal. Consider communication changes, transitioning to the new EHR, and managing resources (human, fiscal, and health care resources).
    5. Discuss what professional, ethical, and regulatory standards must be incorporated into the design and implementation of the system.
    6. Describe the measures and steps you would take to evaluate the success of the EHR implementation from a staff, setting, and patient perspective to measure effectiveness and ensure continuous quality improvement in practice?
    7. Explain what leadership skills and theories would be needed to facilitate collaboration with the interprofessional team and provide evidence-based, patient-centered care?

You are required to cite 5-10 sources to complete this assignment. Sources must be published within the last 5 years and appropriate for the assignment criteria and nursing content.

Complete the “APA Writing Checklist” to ensure that your paper adheres to APA style and formatting criteria and general guidelines for academic writing. Include the completed checklist as an appendix at the end of your paper.

Prepare this assignment according to the guidelines found in the APA Style Guide, located in the Student Success Center.

This assignment uses a rubric. Please review the rubric prior to beginning the assignment to become familiar with the expectations for successful completion.

You are required to submit this assignment to LopesWrite. A link to the LopesWrite technical support articles is located in Class Resources if you need assistance.

  • Business case for quality

this is the pitch : Problem Identification Patient falls remain an ongoing safety concern in the acute care setting, particularly among older adults, patients with impaired mobility or cognition, and those with a history of falls. Falls may result in fractures, head injuries, increased morbidity, and extended hospital stays. Fall-related injuries also increase organizational costs due to additional treatment, prolonged length of stay, and decreased reimbursement associated with hospital-acquired conditions. Current Situation Current fall-prevention measures include risk assessments, bed alarms, and patient education. However, inconsistent reassessment, alarm fatigue, and competing clinical demands may limit the effectiveness of these interventions, allowing preventable falls to continue. Evidence-Based Solution An evidence-based strategy to reduce falls is the implementation of structured hourly rounding using the 4 Ps (Pain, Potty, Position, and Possessions) for high-risk patients. Purposeful rounding proactively addresses patient needs, reduces unassisted ambulation, and improves response time. Research demonstrates that structured rounding significantly decreases fall rates and improves patient satisfaction in acute care settings (Hicks, 2015; Meade et al., 2006). Impact of Policy Change Implementing hourly rounding is expected to decrease fall incidence, enhance patient safety, and improve satisfaction scores at the unit level. Financially, reducing fall-related injuries may decrease costs associated with treatment, extended hospitalization, and potential reimbursement penalties. Hourly rounding represents a low-cost, high-impact intervention that is feasible to implement within the acute care setting. a few instructions. DO: Make sure it is clear on your slides that you’ve met assignment criteria. Use a microphone or test your sound prior to recording the entire video. Organize the presentation in the order of the directions/rubric. Use charts, tables, and other audiovisual elements as appropriate. Include both in-text citations within the presentation and an APA-formatted reference list on the final slide. DO NOT: Read from your slides. You should be professional but conversational. Use font and color combinations that are difficult to read or hard on the eyes (eg, bright and wacky). Overuse bells and whistles (eg, animations). Use complete sentences for bullets. Use too much information slides. Your speech should illustrate your understanding. Ramble. Abide by the time limit. Over communication only makes you look unprepared.
