this is the pitch : Problem Identification Patient falls remain an ongoing safety concern in the acute care setting, particularly among older adults, patients with impaired mobility or cognition, and those with a history of falls. Falls may result in fractures, head injuries, increased morbidity, and extended hospital stays. Fall-related injuries also increase organizational costs due to additional treatment, prolonged length of stay, and decreased reimbursement associated with hospital-acquired conditions. Current Situation Current fall-prevention measures include risk assessments, bed alarms, and patient education. However, inconsistent reassessment, alarm fatigue, and competing clinical demands may limit the effectiveness of these interventions, allowing preventable falls to continue. Evidence-Based Solution An evidence-based strategy to reduce falls is the implementation of structured hourly rounding using the 4 Ps (Pain, Potty, Position, and Possessions) for high-risk patients. Purposeful rounding proactively addresses patient needs, reduces unassisted ambulation, and improves response time. Research demonstrates that structured rounding significantly decreases fall rates and improves patient satisfaction in acute care settings (Hicks, 2015; Meade et al., 2006). Impact of Policy Change Implementing hourly rounding is expected to decrease fall incidence, enhance patient safety, and improve satisfaction scores at the unit level. Financially, reducing fall-related injuries may decrease costs associated with treatment, extended hospitalization, and potential reimbursement penalties. Hourly rounding represents a low-cost, high-impact intervention that is feasible to implement within the acute care setting. a few instructions. Clinical Judgment Process

Recognize Cues

Based on available client data, identify relevant or important cues or information. Subjective and objective could be considered. Specific client values are included where appropriate (i.e., heart rate, oxygen saturation, etc.).

Analyze Cues

Link identified cues to the clients presentation. Determine conditions that are consistent with those cues.

A condition is defined as a normal state with regard to ones health, such as pregnancy, or to a disease, disorder, illness, or injury (National Institutes of Health, n.d.).

Prioritize Hypotheses

Think Where do I start? Based on your analysis of the clients condition, determine which explanations are most likely. List and rank hypotheses according to most urgent problems and priority order.

Generate Solutions

Using the hypothesis, identify desirable outcomes for the client. Goals are listed in SMART format.

In SMART format, goals are Specific, Measurable, Achievable, Relevant, and Time-Bound. Visit the ELO for more information.

Example of a SMART goal:

The patient will report a pain level of 4 or less on a 010 scale within 30 minutes of pain medication administration.

Take Action

Using the identified SMART goals, identify and describe how nursing interventions will be performed, administered, communicated, or taught. Interventions will address the priority concerns and are designed to help meet the clients goals.

Intervention #1 should be crafted to help meet SMART goal #1.

Evaluate Outcomes

Compare observed outcomes against expected outcomes. Determine if your identified interventions were effective.

Evaluative statements must be written in past tense.

Format example:

The (goal was achieved) by (time specified) as evidenced by (supporting findings) after (nursing intervention was performed).

To reiterate: SMART goal #1, Intervention #1, and Evaluation #1 must all be related. This process should be repeated for all goals and their associated interventions and outcomes.

5. Give Credit to Your Sources

a. After completing your concept map, give credit to your sources on both the Reference page and as in-text citations.

Purpose: Plan, implement and disseminate a Community Health and Population-Focused Health Promotion Activity to potential stakeholders, collaborators or supporters.

All parts of the project must be completed in order to receive a grade for this assignment.

PART 1

Plan your health promotion activity/service

Instructions: For this part of the activity, you will begin to plan out your health promotion activity/service by performing a community assessment, research data and trends and selecting a community agency to collaborate with.

1. Each student will use a community assessment model (e.g. Community as Partner) to do an abbreviated community assessment, which must include data collection and a windshield survey, of your selected community.
2. A complete windshield survey should include elements such as boundaries, housing and zoning, open space, commons, transportation, social service centers, stores, condition of the area, race and ethnicity, religion, health indicators, politics, media, media, business and industry.
3. The student will analyze data looking for trends showing strengths, weaknesses, and conditions
4. The student identifies one or two community health problems which represent a particular aggregate at-need or at-risk.
5. Identify an activity or service you can provide to meet this need and your target audience
6. The student will then collaborate with a community agency to plan and implement a health promotion project to address the problems and needs of the aggregate. This can be a church, educational facility, community center, or community group in your community
