Ms. L was referred to you by her primary care provider for Lifestyle Medicine Coaching. As the assigned Lifestyle Medicine Coach, you prepare for your visit with Ms. L by reviewing the following intake assessments: a) Ms. L.’s ; b) c) Ms. L’s , and d) Ms. L’s .

Instructions:

Upon careful review, please answer the following questions:

1. Describe Ms. L’s current health status based on her Biometric Screening. More specifically, answer the following:

  • What is Ms. L’s BMI? Based on her calculated BMI, what is her BMI Classification?
  • What is Ms. L’s Waist to Hip Ratio? What is her risk of heart attack/stroke based on her Waist to Hip Ratio?
  • Based on Ms. L’s blood pressure, how would you define her risk of hypertension?
  • What do the results of Ms. L’s cholesterol screening tell you about her risk for chronic disease?
  • What do the results of Ms. L’s A1C test tell you about her risk for diabetes?

2. Describe the findings of Ms. L’s long form, physical activity and PAR-Q assessments in each of the categories listed below AND compare them to the recommendations/guidelines that you learned throughout the course for preventing chronic conditions (use in-text citations* from your textbook to support recommendations/guidelines):

  • sleep
  • nutrition
  • weight management
  • exercise (and PAR-Q)
  • mental health
  • purpose and connection
  • smoking and substance use
  • medical symptoms

2. Based on all if the information and skill you have obtained from this course, describe how each of Ms. L’s currently lifestyle behaviors may be connected to her current health status (i.e., biometric results) and/or increase her risk for chronic health conditions. 3. Based on the findings of Ms. L’s assessments, what would you advise for Ms. L. with regard to her current lifestyle behaviors (identified in question 2)that can be modified to improve her current health status (described in question 1) ? NOTE: Use in-text citations* from your textbook to support your recommendations.

4. What would be an example of a SMART FITT goal that you could co-create with Ms. L.?
